This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for quote (RFQ) and it does not obligate the Government to award a contract. Request for a solicitation will not receive a response. The purpose of this sources sought announcement is for market research to make the appropriate acquisition decisions and to gain knowledge of potential qualified sources and their Small Business Administration (SBA) size classification in relation to North American Industry Classification System (NAICS) 561320 Temporary Help Services. The Department of Veterans Affairs, Network Contracting Office (NCO) 22 is seeking sources to provide the following professional services to work at the VA Long Beach Healthcare System (VALBHS) Radiation Oncology Department: One (1) Board Certified Senior Therapeutic Medical Physicist One (1) Board Certified Junior Therapeutic Medical Physicist One (1) Board Certified Medical Dosimetrist (CMD) The VALBHS is located at 5901 E. 7th St, Long Beach, CA 90822. The Physicists and CMD will perform all services required in the Performance Work Statement (PWS). The PWS is attached to this notice for download and review. Furthermore, the Physicists and CMD must meet the minimal qualifications and experience to be deemed capable of performing on this contract. The list of qualifications can also be found in Section 2 Qualifications of the PWS. All interested offerors who are capable of providing the qualified Physicists and CMD shall provide a written response to this notice and include the following: Company name, Address, Point of contact, Phone, fax, and email, DUNS number, Cage Code, Tax ID Number, Type of small business, e.g. Services Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), 8(a), HUBZone, Women Owned Small Business (WOSB), Small Disadvantaged Business, or Small Business [NOTE: If you are a SDVOSB or VOSB, you must submit your certification from the Center for Verification and Evaluation (CVE) (https://www.vip.vetbiz.gov/)], Provide a copy of your company s current Representations and Certifications from www.sam.gov, Provide evidence your firm is capable of providing Physicists and CMD with the level of experience as noted in this notice. Firm s shall provide the following information on current and/or previous contracts in which you provided Physicists and CMD (Please note, contract must be within the last three (3) years). Furthermore, the Government may reach out to your listed point of contact to verify information provided in your response: Contract number Facility s Name and Address Provide evidence the facility is ACR accredited. Please use this link: http://www.acraccreditation.org/accredited-facility-search. Point of contact, to include name, title, phone number, and email of the facility in which you have contracted with. Must complete and sign the following Word Documents attached to this notice: S02 CMD QUAL LIST CMD & TMP. S02 JUNIOR TMP QUAL LIST CMD & TMP. S02 SENIOR TMP QUAL LIST CMD & TMP. If possible, provide current hourly rate for the Physicists and CMD at this level of experience and within this geographic location. All responses must be received no later than November 21, 2016, 12:00 PM Pacific Standard Time (PST).
