- conducting a market survey to determine the availability and technical capability of qualified small businesses, including 8(a) small businesses, small disadvantaged businesses, woman-owned small businesses, service-disabled veteran-owned small businesses or HUBZone small businesses capable of providing a fixed priced one year service agreement for the following Illumina instruments: 1. Comprehensive Plan for NextSeq® 500 to include: full coverage for parts, labor and travel; Reagent replacement upon Hardware failures: 1PM; Remote Technical Support; Service technician/personal shall acknowledge repair request w/in 24 business hours and On-site responses shall occur w/in three business days when required, Hardware/Software updates: On-site applications support; advanced training, 5x18 phone support and 5x24 email access to Technical Support. Replacement parts and software upgrades provided must be Original Equipment Manufacturer (OEM) certified. Coverage period start date TBD. 2. Product Care Basic Plan for NeoPrep TM to include: full coverage for parts, labor and travel; Reagent replacement upon Hardware failures: Remote Technical Support; Service technician/personal shall acknowledge repair request w/in 24 business hours and On-site responses shall occur w/in five business days when required, Hardware/Software updates: On-site applications support; advanced training, 5x18 phone support and 5x24 email access to Technical Support. Replacement parts and software upgrades provided must be Original Equipment Manufacturer (OEM) certified. Coverage period start date TBD. Interested firms with the capability of providing/performing, at a minimum fifty-one percent (51%), of the required service, shall submit a capability statement to assist the Government in determining in accordance with Federal Acquisition Regulation (FAR) 19.502-2(b)whether or not this procurement will be set-aside for any of the programs described above. The intended procurement will be classified under North American Industrial Classification (NAICS) code 811219 with a size standard of $20.5M. All respondents are requested to identify their firm's size and type of business. Interested firms responding to this market survey must provide (a) capability statement demonstrating their experience, skills and capability to fulfill the Government's requirements for the above. The capability statement shall be in sufficient enough detail, so that the Government can determine the experience and capability of your firm to provide the service above. Capability statements must include the name and telephone number, email address of a point of contact having authority and knowledge to discuss response with Government representatives. Capability statements in response to the market survey that do not provide sufficient information for evaluation will be considered non-responsive. A copy of the capability statement must be received at the contracting office address/designated location: 6710B Rockledge Drive, 1159B, Bethesda, MD 20892 - ATTN: Tina Robinson or sent via email to robinsti@mail.nih.gov prior to 10:00 a.m. eastern standard time on November 23, 2016. Capability statement must reference solicitation# NICHD-17-015. Submission by email must also reference solicitation # NICHD-17-015 on the subject line of the email.
