- Combined Synopsis Solicitation Instrument Set Copy MS Word This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the information in FAR Subpart 12.6, using Simplified Acquisition Procedures for commercial items found at FAR 13, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This combined synopsis/solicitation SHALL be posted on FEDBIZOPPS. Naval Medical Center San Diego (NMCSD) has a requirement for five (5) instrumentation sets that will be used in cardiothoracic surgery procedures. The instrument sets are specific in order for the surgeon to maneuver the instrument through delicate tissues and perform the required action of that instrument set. The specific need is for instrumentation that meets the following criteria: 1. All items must come from the same manufacturer. These items are used in cardiothoracic emergency procedures, changing items or having instruments that are not compatible can cause delay to the surgery and in turn cause harm to the patient. They will create 5 identical sets to be used interchangeable; there is no room for variation with these instruments. 2. Items must be coated with super grip polymer to prevent instruments from slipping in the surgeons hand during the procedure. The Request for Quotation (RFQ) number is N0025917Q0034. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-91 and DFARS Publication Notice 20161021. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The NAICS code is 339112 and the Small Business Standard is 1000. This is a competitive, 100% small business set-aside solicitation. The Small Business Office concurs with the set-aside determination. The Naval Medical Center San Diego (NMCSD) requests responses from qualified sources capable of providing the restock of the following items that must meet the salient characteristics described above: CLIN Description Qty Unit Price 0001: Jacobson Micro Scissor: must be round handle with 7mm 5 $ scissor blade length, be 18cm in length total, with a 60 DEG angle. 0002: Jacobson Micro Scissor: must be round handle with 7mm 5 $ scissor blade length, be 18cm in length total, with a 125 DEG angle. 0003: Jacobson Micro Needle Holder: must be straight needle 10 $ holder with round handle with locking mechanism, 18 cm in length, with a straight angle of tungsten carbide 0004: Jacobson Micro Needle Holder: must be straight needle 10 $ holder with round handle with locking mechanism, 22.5 cm in length, with a straight angle with diamonite coating to the tip of the instrument to grip the suture being used. 0005: Vascular Dilator Memory Shape: a vascular dilator made 5 $ of Nitinol that is 14cm in length and 2.0mm in diameter. Must be made with Nitinol to allow for the item to be bent and manipulated but resume original position; this way the dilator does not get or stay bent causing damage to the vessel it is needed to dilate.
