Description
Seeking As-Needed Primary Care Physicians Captain James A. Lovell Federal Health Care Center The Great Lakes Acquisition Center (GLAC), on behalf of Captain James A. Lovell Federal Health Care Center (FHCC) located at 3001 Green Bay Road, North Chicago, IL 60064, is seeking services of Board Certified/Board Eligible-Internal Medicine/Family Practice Physicians for its Primary Care Directorate. The intent of this Sources Sought Notice is to identify potential vendors that would be interested and capable of providing a pool (at least three) of Internal Medicine and/or Family Practice Physicians to provide services on an AS NEEDED basis at the FHCC and/or any of its three Community Based Outpatient Clinics (CBOCs) Evanston CBOC, Kenosha CBOC, and McHenry CBOC. These CBOCs are located at 1942 Dempster Street in Evanston, IL 60202; 8207 22nd Avenue, Suite 150 in Kenosha, WI 53143; and 3715 Municipal Dr in McHenry, IL 60050, respectively. When the need for Physician Services at the FHCC arises, which will typically be to cover vacancies within the Patient Aligned Care Team (PACT), the required period of performance will range from 2 to 32 consecutive weeks. The required duty hours are Monday through Friday from 8:00 a.m. to 4:30 p.m. The FHCC will not require weekends, after normal duty hours, or Federal Holidays. It is the intent of the FHCC to establish two-year Blanket Purchase Agreements (BPA) with multiple vendors in accordance with FAR 13.303. A BPA is not a contract, but is a simplified method of filling repetitive needs for supplies and services with qualified vendors. A BPA contains (1) the terms and conditions applying to future orders between the parties during the term of the BPA, (2) a description of the services to be provided, (3) pricing, and (4) methods for issuing and accepting future orders under the BPA. The establishment of a BPA does not obligate any funds or guarantee or imply that any orders for services will be placed. Upon establishment of the BPA, each identified physician in the pool shall undergo a mandatory Access National Agency Check and Inquires (ANACI) Background Investigation (BI). Concurrently with the BI, the FHCC will conduct the credentialing and privileging process. Physicians cannot perform any services at the FHCC prior to being both successfully adjudicated and granted privileges. The current fees associated with the ANACI background investigations are $645.00 per individual. This Sources Sought Notice is for market research purposes only and shall not be considered as a request for quote or as an obligation on the part of the Government to acquire any services. No entitlement to payment of direct or indirect costs or charges by the Government will arise as a result of the contractor submission of this Sources Sought Notice or the Government's use of such information. The Government reserves the right to reject, in whole or in part, any contractor's input resulting from this Sources Sought Notice. No contractual arrangement will be made as a result of this announcement. Information submitted in response to this Sources Sought Notice will not be returned. All interested parties that are capable of providing the required services shall submit a response via email to darryl.scherer@va.gov and elester.wilbern2@va.gov no later than November 30, 2016 at 10:00 a.m. central time. The responses shall address the information below. Provide a list of potential Physicians who are interested and available to be part of the approved Physician pool anticipated to start February 1, 2017. Indicate whether or not they have prior VA/DOD experience. Provide CVs for all physicians identified on the list above. Provide a brief description of the company background, capabilities relating to this requirement, socioeconomic status, past performance with Government contracts, and contractor DUNS number. Indicate the required minimum advanced notice needed to fulfill a physician request (i.e., 48 hours, 1 week, etc.). Provide an all-inclusive estimated hourly rate for physician services required above (Internal Medicine and/or Family Practice and travel/non-travel provider. Provide proof of VET-biz (VIP) verification if claiming Veteran-Owned Small Business or Service-Disabled, Veteran-Owned Small Business socioeconomic status. Provide a copy of current certificate of liability insurance. All information should be UNCLASSIFIED or NON-PROPRIETARY.
