Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. SYNOPSIS: The Department of Veterans Affairs, VISN 6 Network Contracting Office, on the behalf of the Durham VA Medical Center has a need for a Preventative Maintenance Service agreement on Newport Medical HT70 and HT50 ventilators. Offeror must be an authorized service provider for Newport Medical/ Covidien equipment with access and keys to equipment software and software upgrades. See Statement of Work in the attached solicitation for full description of requirement. SOLICITATION: See attached solicitation VA246-17-Q-0022. This solicitation is a small business set-aside. Only small businesses will be considered for award. The North American Industry Classification System (NAICS) Code for this acquisition is 811310, Size Standard: $7.5 M, and the Classification code is J065. Vendor must be registered in SAM (System for Award Management). Lack of registration in the SAM database will make an offeror ineligible. To register, vendors may go to https://www.sam.gov via the internet. Quotations shall include the RFQ number, the proposed price for each contract period, company name, address, phone number, fax number, tax identification number, DUN number, and size status (example: small veteran owned business, small disadvantaged business, small woman-owned business). The Government shall consider quotes received by 8:00 AM Eastern Standard Time, November 22, 2016. Quotes are to be e-mailed to dori.dabbs2@va.gov.
