Quote Sheet Tentative Schedule Quality Assurance Surveillance Plan Statement of Work THIS ACQUISITION IS A 100% SMALL BUSINESS (SB) SET-ASIDE. All responsible SB sources may submit a bid, proposal, or quotation which shall be considered by the agency. The Air Force Sustainment Center Operational Division (AFSC/PZIOA) located at Tinker Air Force Base Oklahoma, intents to award a Firm Fixed Price (FFP) contract for Aerobics Instructors being provided to the Gerrity Sports & Fitness Center. The period of performance is 15 Dec 16 - 14 Dec 17 with four (4) sequential option years. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Please provide a Monthly Firm Fixed Price bid, proposal, or quoation that will incorporate any combination of the classes described in paragraph 2 of the SOW. The implementation plan should also include a proposed monthly schedule of classes that incorporate a combination of classes specified in the SOW, along with the number of employees that will be committed to this contract as well as all of their certifications. (See attached tentative schedule). Contractor is required to submit the attached quote sheet with proposal submission. SERVICES NON-PERSONNEL: To furnish all labor, transportation, and incidentals as necessary to provide the Gerrity Sports & Fitness Center Aerobic Instructional Services in accordance with task identified herein. Period of Performance: 15 DEC 16 - 14- DEC 17 with four (4) sequential option years. 2. INSTRUCTOR TERMS: Contractor shall teach classes as scheduled by the Fitness Center Director from the following: low impact, mid impact, high impact, step, floor aerobic, yoga, kick boxing, Pilates, circuit training, turbo kick, cardio party, power pump, spinning, Zumba and strength, water aerobics, etc., classes. Instructor shall teach methods that provide aerobic fitness to individuals with beginner, intermediate, and moderate fitness levels. Personal assistance and corrective techniques will be provided before and during classes. Contractor shall provide preparation to include musical device setup and ensure musical device has been secured after use. Contractor shall ensure classes taught as part of AF Fitness Program meet requirements as establish by Health and Wellness Center (HAWC) and Fitness Center Director (FCD). Contractor shall provide FCD with daily attendance sheet for each class detailing number of participants and status (Active Duty Air Force or Other).. The contractor shall furnish all labor transportation, tools and incidentals as necessary to provide service. Re-performance of service shall be administered for any missed classes or contracted service as stated in the FY 17' awarded contract by vendor to customer w/in three (3) business days as stated in the QASP. 3. CERTIFICATION: Applicant shall be knowledgeable regarding current fitness and exercise techniques and have a current certification in one of the nationally recognized fitness agencies or authorized substitute. Copy of certification(s) and CPR certificate(s) shall be submitted prior to time of award. Instructors shall have current CPR certification.. 4. PERFORMANCE: Contractor shall be courteous, use constructive criticism, and never insult class participants; and shall be able to explain, demonstrate, and perform all aerobic exercises being taught. Contractor shall stay up to date on all new aerobic moves and guarantee a variety of routines, and must be proficient in and familiar with hand held exercise equipment( dumbbells, body bars, exercise balls, steps w/ risers, barbells, exercise bands, etc.). 5. QUARTERLY MEETINGS: Contractor shall meet with Fitness Center Director or his/her representative to discuss/review the status of Aerobic Class and exchange feedback for remaining period. 6. DATE and TIMES: Classes are to be offered 15 times per week Monday - Friday as scheduled and coordinated with the Contractor by the FCD at the Gerrity Sports & Fitness Center. Instructor shall be prepared for class 10 minutes prior. Classes will not be offered on federal holidays. Contractor may be required to provide Aerobic instructions at an alternate site, located on TAFB. Sessions maybe canceled by Gov't on an individual basis 24 hours in advance of the scheduled class. Classes may be re-arranged by the request of the Contract Monitor during the time frames of 0700-0800, 0900-1000, 1000-1100, 1200-1300, 1300-1400, 1500-1600 and 1600-1700. 7. COST of CLASS: There will be no charge for classes. Mandatory participants will have priority. Contractor shall submit accurate invoice within 5 working days following last day previous month to the Fitness Center Director or Fitness Director. 8. COORDINATION REQUIREMENTS: Contractor may be required to coordinate activities with Fitness Center Director or his/her representative. Contractor may be required to work with other Aerobic Instructors or Fitness Center Director or his/her representative or Fitness Center Staff in the performance of this statement of work. 9. CLASS FORMAT: Class format will be followed and instructor shall encourage heart rate monitor use when needed. Classes designated for AF Fitness Program must meet guidelines and standards established by HAWC and Fitness Center Director. a. Aerobic Phase: Low impact aerobic activity sufficient to keep participants' heart rate within their Training Heart Rate Zone. Aerobic phase will last at least 30 minutes but not exceed 40 minutes. b. Cool Down: A gradual cool down that allows participants the ability to return to their pre- exercise exertion levels i.e. "conversation exercise". Cool down should last 6-8 minutes followed by 7-9 minutes of stretching. Class format for AF Fitness Program classes: a. Warm-up: A gradual warm-up that allows body to prepare for the stress of exercise by increasing blood flow, muscle temperature and must include stretching for major muscle groups. b. Cardiovascular Phase: Cardiovascular phase will be sufficient enough to ensure participants heart rates are within 70-85% of maximum heart rate. Instructor must prompt participants to check heart rate every 10-15 minutes. Cardiovascular phase shall be between 30-40 minutes, rhythmic in nature, continuous and involve large muscle groups. c. Strength Phase: Last 15-20 minutes, include at a minimum, 1 set of 8-12 repetitions of exercises or movements that involve all major muscle groups. d. Cool Down: A gradual cool down that allows participants the ability to return to pre-exercise exertion levels where "conversation exercise" is established and includes 7-9 minutes of stretching. 10. EQUIPMENT SUPPLIED BY CONTRACTOR: Aerobic music. 11. EQUIPMENT SUPPLIED BY GOVERNMENT: Sound system, steps, bands, hand weights, balls. 12. WORK SCHEDULES: Monthly work schedule will be submitted to Fitness Center Director or his/her representative no later than the 3rd of each month for the following month. Contractor is to ensure all classes and times are covered with qualified aerobic instructors as described here within. Contractor shall always have backup instructor on call should the scheduled instructor fail to show for the class. 13. INSPECTION and ACCEPTANCE of SERVICES: Aerobics instructors will be monitored on a daily basis for compliance with responsibilities detailed within statement of work. Such as: a. Present and on time b. Uses equipment properly c. Follows class format d. Provides point of contact e. Submits accurate invoices on time f. Certified by national or authorized organization g. Meets ALL terms of statement of work not specified above 14. SERVICE DELIVERY SUMMARY. SURVEILLANCE: Items that require inspection will be periodically monitored after completion of work to assure that the required service has been accomplished IAW the contract. ******************************************************************************************** QUALITY ASSURANCE SURVEILLANCE PLAN FOR AEROBICS INSTRUCTOR This Quality Assurance Surveillance Plan (QASP) has been developed to evaluate Contractor actions while implementing this Statement of Work (SOW). It is designed to provide an effective surveillance method of monitoring contractor performance for each listed objective on the Service Delivery Summary (SDS). The QASP provides a systematic method to evaluate the services the Contractor is required to furnish and not the details of how the Contractor accomplishes the work. This QASP is based on the premise that the Government desires to maintain a quality standard in providing Aerobic Instruction. A service contract to provide this service is the best means of achieving this goal for Fitness Center Customers. The Government retains the right to inspect any item included in the contract, including tasks not shown on the SDS. These services should be inspected in the same general manner as periodic surveillance items. The results of these inspections are documented and, if necessary, provided to the Contracting Officer (CO) for action. Should a discrepancy be observed, the CO will handle each documented discrepancy on a case-by-case basis. The contractor, and not the Government, is responsible for management and quality control actions to meet the terms of the contract. Good management and use of an adequate control plan will allow the contractor to operate within specified performance requirements. The role of the Government is quality assurance to ensure contract standards are achieved. In this contract, the quality control program is the driver for product quality. The contractor is required to develop a comprehensive program of inspections and monitoring actions. The first major step to ensuring a "self-correcting" contract is to ensure that the quality control program approved at the beginning of the contract provides the measures needed to lead the contractor to success. Once the quality control program is approved, careful application of the process and standards presented in the remainder of this document will ensure a robust quality assurance program. If inspection indicates unacceptable performance, the QAP will notify the contractor of the deficiencies for correction. The contractor shall be given a reasonable time (3 DAYS) after notification to correct the unacceptable performance if such correction is possible. The length of time allowed to correct the problem will depend upon the requirement and the deficiency. The contractor will be notified of the time allowed for correction when the deficiency is reported to the contractor. The QAP will inspect and accept contractor services at the completion of each contract payment period. The QAP will certify services actually received under this contract via DD Form 250, a letter to the C O, and or contractor's invoice.
