VA261-17-B-0107 Response Due 12/06/2016 3:00 PM Pacific Standard Time Page 1 of 1 This is a Sources Sought for information and planning purposes and shall not be construed as an obligation on the part of the VA. The purpose of this sources sought is to identify qualified businesses, including Service-Disabled Veteran-owned Small Business (SDVOSB) Veteran-Owned Small Business (VOSB), small disadvantaged businesses (SDB), Women-owned Small Business (WOSB), Historically Underutilized Business Zone (HUBZone) Small Business, 8(a) Small Business and small business concerns that are interested in and capable of performing the work described herein. The NAICS code is 541380 and the corresponding size standard is $15.0 Million. Your responses to the Request for information will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. The VA Sierra Nevada Health Care has a requirement to hire a concrete testing firm to execute the following tests on a floor for the installation of medical equipment. The tests are required by the equipment manufacturer as follows: a. Surface Non-Destructive Concrete Moisture Detector ASTM F-2659 (Tramex CME4) <3lbs and <75% moisture vapor transmission. b. Relative Humidity Probe ASTM F2170 (Wagner Rapid RH) c. Floor flatness 2 X2 grid showing elevation provided by the general contractor. d. Concrete elevation in relation to top of Operating Room table base plate. e. Swiss Hammer Test ASTM C805 to be 4300 PSI or third party testing reports at time of pour. f. Pull-off test tobe1.5MPa Interested firms fitting one or more of these Socio-economic categories should send their information by E-mail to charee.harris@va.gov no later than 3:00 PM Pacific Standard Time, December 6, 2016. The following information is requested in response to this notice: (1) Type of socio-economic category (e.g. SDVOSB, Small Business, etc.), if firm is a SDVOSB or VOSB, evidence of verification in VetBiz as a SDVOSB or VOSB (SDVOSB and VOSB firms must be verified by the Center for Veteran Enterprises (CVE); (2) A positive statement of your intention to submit a bid in response to a solicitation for this project. All information must be submitted in sufficient detail for a decision to be made on the availability of interested firms fitting one of more socio-economic categories. If adequate interest is not received, this action will not be set aside for SDVOSB, VOSB, WOSB, HUBZone, 8(a) small business or small business firms.
