Description

The Department Of Veterans Affairs, Network 15 Contracting Office has an EMERGENCY PROCUREMENT REQUEST for the replenishment of the Medtronic Minimeds insulin pump supplies. This request utilizes Brand Name Brand Specific Only IAW FAR 6.302-1(c)(1), with No Substitutions Allowed as specified below. This request also requires the Urgent Delivery with the (DLT) Delivery Lead Time of 5 days ARO located at the (VA) Leavenworth CMOP located at 5000 S. 13th Street Leavenworth, KS 66048-5580. This delivery requirement is due to the Leavenworth CMOP currently has one item that is out of stock and another one that is very low. If the MINIMED supplies listed below are not received in a timely manner/delivery, will cause patient harm and could prove to be directly threatening to the life and health of the Veterans (patient). The Government anticipates making one or more firm-fixed-price awards. All responsible sources may submit a quotation, which if timely received, shall be considered by this agency.
