Description
Synopsis: Veterans Health Affairs, Network Contracting Office 12, is issuing this request for information/sources sought notice in order to identify potential sources that have the resources & capabilities to provide Cardiothoracic Physician Assistant services for the Clement J. Zablocki VA Medical Center, Milwaukee, WI. Scope of Work: The contractor shall provide Cardiothoracic Physician Assistant(s) (PA) to perform services at the Clement J. Zablocki VA Medical Center, 5000 West National Avenue, Milwaukee, Wisconsin. The VA Chief of the Cardiothoracic Surgery Section will provide technical and professional direction. Other members of the Cardiothoracic Surgery Section attending staff are designated alternates. The contractor shall provide approximately 24 hours per week (Monday through Friday between the hours of 6:00am to 6:30pm) of Cardiothoracic PA services. These services would be scheduled a week in advance and provided to the potential awardee by the VA Surgery Scheduler. Outside of the scheduled surgeries the contractor shall be responsible for providing on call coverage 24 hours a day, 365 days per year. On-call means the scheduled PA will carry a pager and is located within 30 minutes of reporting to the VA Medical Center. On-call services would include federal holidays. While on-call at the Milwaukee VAMC, the PA s covered by this contract are not to be concurrently on-call to both the VA and another institution for the same periods of time. Alternatively, the contractor will be allowed to create a call schedule whereby there is a primary PA on call with a second PA on secondary call, available for the VA thus always having someone available for call. The VA anticipates a provider would need a rotating pool with a minimum of 6-8 Cardiothoracic PA s, to cover scheduled surgeries and on-call/call-back requirements. Cardiothoracic PA Qualifications: Cardiothoracic Physician Assistants shall have a full and unrestricted license in a State, Territory, or Commonwealth of the United States or the District of Columbia. The Physician Assistant must hold credentials from the National Commission on Certification of Physicians Assistants, Inc. (NCCPA) and the Wisconsin Department of Regulations and Licensing or other comparable state license. Physician Assistants shall have a minimum of one year documented experience as a certified cardiac physician assistant and: A bachelor s degree from a PA training program which is certified by the Accreditation Review Commission on Education for Physician Assistants (ARC-PA) or Graduation from a PA training program of at least 12 months duration which is certified by the ARC-PA and a bachelor s degree in a health care occupation or a health related science or Graduation from a PA training program of at least 12 months duration which is certified by the ARC-PA and a period of progressively reasonable health care experience such as an independent duty medical corpsman, licensed practical nurse, registered nurse, medical technologist or medical technician. The duration of approved academic training and health care experience must total at least 5 years. This Request for Information/Sources Sought will assist the contracting office in its market research, and provide insight into potential qualified vendors to include Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HUBZone and other Small Businesses interested and capable of performing the work. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) company name b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) business type (i) socio-economic status, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUBZone, Women Owned Small Business, Small disadvantaged business, or Small Business HUBZone business (j) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described above (k)curriculum vitae of cardiothoracic PA s meeting qualifications specified above, and (l) estimated hourly rates for cardiothoracic PA s. Responses must be submitted by 4:00 PM (CST) December 7, 2016. The results of this market research will assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., small business set-aside, full and open competition, etc.).
