Chapter VI: Other Than Full and Open Competition (OFOC) SOP Attachment 4: Single Source Justification for SAP under $150K OFOC SOP Revision 05 Page 1 of 1 Original Date: 03/22/11 Revision 05 Date: 05/02/2016 JUSTIFICATION FOR SINGLE SOURCE AWARDS IAW FAR 13.106-1 (OVER MICRO-PURCHASE THRESHOLD($3.5K) BUT NOT EXCEEDING THE SAT ($150K)) 1. ACQUISITION PLAN ACTION ID: VA259-17-AP-0997 1A. PROJECT/TASK No. VA259-17-D-0035 1B. ESTIMATED AMOUNT: $132,562.50 2. BRIEF DESCRIPTION OF SUPPLIES OR SERVICES REQUIRED AND THE INTENDED USE: These services are currently being performed by Colorado Western Healthcare located at 2021 N 12th St., Grand Junction, CO 81501 under VA259-16-D-0036 which will expire on November 30, 2016. The new period of performance will be December 1, 2016 April 30, 2017. The IDC will cover services until a new, competitive action is awarded. The total estimated value of this acquisition is $132,562.50 which is based on historical pricing. 3. UNIQUE CHARACTERISTICS THAT LIMIT AVAILABILITY TO ONLY ONE SOURCE, WITH THE REASON NO OTHER SUPPLIES OR SERVICES CAN BE USED: VA Surgeons are currently performing Intermediate Complexity Surgeries at the Colorado Community Hospital. After November 30, 2016, the Contractor will have no obligation to allow VA Surgeons to perform services at their site. Without a surgical suite, VA cannot perform intermediate complexity surgeries and patient services would be inadvertently impacted. Colorado Community Hospital has a facility that is FDA compliant. The facility has also already Credentialed and Privileged VA Surgeons. Another facility would need approximately 30-6- days to Credential and Privilege VA surgeons. This would also inadvertently impact patient care. 4. DESCRIPTION OF MARKET RESEARCH CONDUCTED AND RESULTS OR STATEMENT WHY IT WAS NOT CONDUCTED: A new contract is currently being solicited for competitive award. Due to CRB approvals and revised requirements, this new contract will not be in place by December 31, 2016. 5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b).
