- THIS IS A SOURCES SOUGHT NOTICE ONLY (Revised). THIS IS NOT A REQUEST FOR PROPOSAL. The Department of Veterans Affairs George E. Wahlen Health Care System (VASLCHCS), Salt Lake City, UT and the Network Contracting Office (NCO) 19, Glendale, CO are performing market research to determine if there s any qualified (1) Service Disabled Veteran Owned Small Businesses; (2) Veteran Owned Small Businesses; (3) Small Businesses and Large Businesses to compete in a Firm Fixed Price Contract requirement to provide Off-site highly specialized Laboratory and Genetic Reference Testing Services for VASLCHCS. The Government will use responses from this notice to make the appropriate acquisition decisions. The proposed solicitation will be issued in accordance with FAR Parts 12 and 13. The contractor shall be responsible for performing specific genetic and neurologic tests within their organization (no outsourcing test if so, explain cost and time-sensitivity) that are required by the Genomic Medicine and Neurology Services for critical laboratory analysis and result interpretation for veterans patient care at the VASLCHCS. The services shall include the performance of analytical testing, and the reporting of analytical test results to include consultative services as required to assimilate the full scope of patient care. The required highly specific genetic and neurologic testing is as follows but not restricted too: Test Menu Description Encephalopathy, Autoimmune Evaluation, CSF Encephalopathy, Autoimmune Evaluation, Serum Paraneoplastic Autoantibody Evaluation, CSF Paraneoplastic Autoantibody Evaluation, Serum Neuromeyeltic Optica Paraneoplastic Autoantibody Amyloid Protein ID Autoimmune Dysautonomia Oxygen Dissociation P50 Erythrocytes Vedolizumab & Anti-Vedolizumab The place of performance for these laboratory services shall be performed at the vendor s primary location (in-house). The period of performance for this contemplated contract action will be for a Base year (12 months), with four (4) additional inclusive option years, in which, the total estimated cost for services rendered for the Base year will be about $50,000.00 per year. The total contract cost including all options (5 years) will be estimated at $ 250,000.00. This contemplated contract requirement is anticipated to be awarded on /or around January 15, 2017. All interested vendors that has these specified laboratory testing servicing capabilities should respond by email, in writing, with information in the form of capability statements describing their qualifications (to include full Test Menu); and also indicate which socioeconomic business category for which they qualify for to the NCO 19 Contracting Officer Edward Marshburn at edward.marshburn@va.gov no later than 4:00 p.m. Mountain Division Time (MDT) Wednesday, December 05, 2016.. The Solicitation (RFP) VA259-17-R-0066 has been assigned and should be referenced on all correspondence regarding this announcement. THIS IS NOT A REQUEST FOR PROPOSAL. The NAICS Code 621511 is applicable to this acquisition with a Business Size Standards amount of $32.5 Mil. Interested vendors are reminded that in accordance with FAR Part 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the System for Award Management (SAM) database prior to award of a contract. The reference laboratory must be licensed and accredited by the College of American Pathologists (CAP), Clinical Laboratory Improvements Amendments and/or other state regulatory agencies as mandated by federal and state statutes N/F EEM
