Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is VA258-17-Q-0044 and is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-89 effective 11/01/16. The associated North American Industrial Classification System (NAICS) code for this procurement is 339113 with a small business size standard of 750 employees. THIS REQUIREMENT IS A SET ASIDE FOR: SERVICE DISABLED VETERAN OWNED SMALL BUSINESS. FOB destination: New Mexico VA Health Care System, 1501 San Pedro Dr SE, Albuquerque, NM 87108-5128. Quotes for the following products are requested and as set forth in the RFQ: two (2) ea, ultrasound storage cabinets for TEE probes, and two (2) ea, replacement filters. Cabinets SHALL be in compliance with: VHA Directive 1116 (2), page 36, paragraph 21.c.8.a. Refer to: block 20 of the SF1449, Statement of Work, and B.3 Price/Cost Schedule of solicitation VA258-17-Q-0044 for information, where to input quoted prices, and dates of delivery requested. ***Question Submission: Interested offerors shall submit all inquiries concerning the RFQ at the earliest possible time to enable the Buyer to respond. Inquiries to be submitted via email to: Bryan.Rogers2@va.gov. Amendments to the RFQ addressing inquiries or concerns will be posted as an amendment to the RFQ on this same website. Questions not received within a reasonable time prior to close of the RFQ may not be considered. *** New Product only; NO remanufactured or "gray market" items. All items shall be covered by manufacturer's warranty. Quote shall be valid for a minimum of 60 calendar days after date of quotation received. This solicitation requires registration by the vendor with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to this RFQ must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov in order to receive an award. Failure to comply with the terms and conditions as stated in this announcement and the RFQ may result in offer being determined as non-responsive: the selected Offeror shall comply with all clauses and provisions as listed in the RFQ.
