SOURCES SOUGHT ANNOUNCEMENT: The Department of Veterans Affairs Lebanon VA Medical Center, 1700 South Lincoln Avenue, Lebanon, PA 17042 is conducting a market survey to identify potential sources to provide Palliative Care Survey Services. This will be set aside for Veteran Owned Small Businesses (VOSB) and Service Disabled Veteran Owned Small Businesses (SDVOSB) This announcement is issued for informational planning purposes only and is not a request for proposal or announcement of a solicitation. Submitting information for this announcement is voluntary and participants will not be compensated. The purpose of this market research is to identify capable sources of providing services which includes the services of contract personnel administer surveys (verbally via phone and via mailed surveys) of the next of kin of deceased inpatient Veterans for the purpose of collecting and documenting these data for VA s Hospice and Palliative Care program as a central indicator of the quality of care delivered. The feedback from these surveys provides the voice of veterans in VA s strategic planning to be a patient-centered healthcare organization. The North American Industry Classification System Code (NAICS) is 561320 and the small business size standard is $7.5 million. In addition to a Capability Statement which provides the above, submissions can include specifications, brochures, and manuals as attachments.
