- Wage Determination Level 2 layout Level 1 layout AIR QUALITY ASSESSMENT AG-9JGP-S-17-0010 December 7, 2016 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is AG-9JGP-S-17-0010 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-92. This is a total small business set-aside. The NAICS code is 541620 and small business size standard is $15M. This is for Air Quality Assessment Statement of Work Building Assessment and Mold Inspection Of the Tahoe National Forest Supervisor's Office Nevada City, CA I. BACKGROUND In late May 2016, the Tahoe National Forest received a health hazard assessment of the Forest Supervisor's Office from the National Institute for Occupational Safety and Health (NIOSH). This review was conducted because of the building's history of water leaks and employee complaints of mold exposure related symptoms. While visual evidence of water damage and mold growth has been observed in several areas of the building, no comprehensive assessment of the extent of the water intrusion and damage and subsequent mold growth has been conducted. Based on the recommendations of NIOSH and the continued employee health concerns, the agency has determined that a comprehensive building wide assessment needs to be performed to determine and document the extent of the water damage and mold growth. The Forest Service intends to utilize this information to work with the building landlord to have mitigation measures implemented II. SITE DESCRIPTION AND LOCATION The Tahoe National Forest Service Supervisor's Office, located at 631 Coyote Street in Nevada City, California. The Supervisor's Office building is a two-story structure, with multiple offices on both floors. The original building was constructed as a slab-on-grade, one-story structure in 1962. Multiple building additions occurred in the late 70's and early 80's which added a second floor and more floor space. There are multiple heating, ventilation, and air-conditioning (HV AC) systems throughout the building. The Supervisors Office contains about 22,000 SF of space over two levels, There are 76 room on the first floor an 46 room on the second floor. The Vicinity map and layout are attached. The construction is primarily light wood framing with the bottom floor Slab On Grade (SOG) and the second floor partially on floor joists and SOG. The attic space is a scuttle attic with trussed roof design. The HVAC system is composed of 7 separate AC and gas furnace units. III. SCOPE The Contractor shall furnish all labor, equipment, transportation, permits, testing and incidentals necessary for the performance of the tasks specified in this Statement of Work. The overall purpose of this work is to provide a comprehensive assessment of the Tahoe National Forest Supervisor's Office to determine and document the extent of water damage and intrusion and mold growth. Key work items include, but are not limited to: A. Conduct an on-site inspection and assessment of the building to identify locations of water intrusion and leakage and areas of moisture and dampness which will promote mold growth. A Minimum of 5 air samples and 5 surface samples shall be taken from representative areas within the building that includes but is not limited to: attic spaces, wall cavities, ceiling and floor crawl spaces, foundation slab, and HVAC system). B. The Results of the inspection and preliminary test samples shall be used to develop a sampling and testing protocol for the building to be delivered in draft form within one week of the site inspection. C. Based on the draft sampling and testing protocol the Tahoe National Forest may retain the option to award the following work items: • Conduct wall cavity sampling in areas with detected moisture and/or mold growth. • Conduct an extensive interior and exterior building wide air sampling and surface sampling of areas throughout the Supervisor's Office, that are identified in the preliminary sampling and testing protocols (viable and non-viable sample analysis). Inspection and surface and air sampling of the heating and ventilation system and ducts. D. Photographic documentation of all inspections. This includes, but is not limited to, wall cavity and HVAC system air duct photo documentation. E. Collection of outdoor background air samples, field and laboratory QA/QC samples. IV. CONTRACTOR QUALIFICATIONS Contractor personnel conducting the building assessment and mold investigation shall hold current and applicable certifications and licenses relevant to the tasks specified in this scope of work. For example, certification through: • The National Organization of Remediators and Mold Inspectors as a "Certified IAQ/Mold Inspector" or "Certified Mold Assessor". • The American Council for Accredited Certification as a "Council-certified Microbial Consultant". • Or equivalent professional organization. V. SITE ACCESS Building access and scheduling will be coordinated with the Forest Service Point of Contact for this contract. Access to law enforcement and secure storage areas will be coordinated with the Deputy Forest Supervisor if required. VI. DELIVERABLES A. Final Report: The Contractor shall prepare and provide three (3) hard copies and an electronic copy of a Final Building Assessment Report no later than 14 days from the completion of performance of work under this contract. The Final Report shall include: i. A detailed description and documentation of the investigation and results. Including, but not limited to: o Description of field methodology. o Description of exposure and evaluation criteria and standards. o Field investigation findings (general observations, locations of water intrusion and observed and potential mold growth, airborne and surface sample results, key areas impacted, etc.). ii. Discussion of recommended mitigation measures. iii. Photo documentation. iv. Copies of all laboratory analytical results. v. Copies of field sampling protocols followed. VII.
