Description

Building 59A Holding Room: AMENDMENT NO. 2 VA244-17-B-0178 This amendment is hereby issued as follows: 1. Include Preconstruction Risk Assessment form. 2. Answers to contractor questions: 1. Shall a camera be installed and what are the specifications of the required equipment? VA Response: Yes, the camera is a Panasonic Model # WV-SF332. 2. Does the sprinkler in 107A need to be replaced with an anti-ligature sprinkler head? VA Response: Refer to the specifications, Sprinkler System: The contractor shall install a new anti-ligature sprinkler system in room 107 and 107A. If the current sprinkler system in room 107A meets the anti-ligature code, then the sprinkler system in that room can stay. The contractor shall test new sprinkler system. 3. Where can we acquire the specifications for all of the proposed equipment (ie. card reader, nurse call button, anti-ligature fixtures etc.? VA Response: Specifications are in the manuals cited in the statement of work. The contractor that we use for the camera and the card reader is M3TCorporation.com. The camera is a Panasonic Model # WV-SF332. The card reader is a XceedID Corporation Model # MT15. The contractor that we use for the nurse call station is Kratosdefense.com. The nurse call station is a Rauland Borg Model# R4KPC11 (pull cord) and R4KPB12 (push button) 4. What is the load rating for the existing HVAC unit? VA Response: The manufacturers website will decipher what this model number means. HVAC unit shall be Trane model FCEB0401 JAAD0A20BF2M0000D3GEEF4000000000000 OR EQUIVALENT in piping or drainage entry and exit and be compatible with existing systems and architecture. Reference Trane Manual UNT-SVX07D-EN. 5. Is ceiling mounted patient lifts included in this project? VA Response: There will be no need for any patient lift apparatus within the scope of this project. 6. Can you provide a manufacturer and model number for the existing security camera, so that we can match it with a compatible security camera? VA Response: The camera is a Panasonic Model # WV-SF332. The contractor that we use for the camera and the card reader is M3TCorporation.com 7. Can you provide a "basis of design" type or model for the anti- ligature light fixtures? VA Response: Existing 4 foot lighting is: Cooper model# FMR D 24 UNV 80/86 MF24-4FT. Efforts shall be made to acquire an LED equivalent even if available in a 2 x 2 equivalent in lumens. 8. Can you provide the name of the company who manages the nurse call system? VA Response: The nurse call station is a Rauland Borg Model# R4KPC11 (pull cord) and R4KPB12 (push button). The contractor that we use for the nurse call station is Kratosdefense.com.
