- SOURCES SOUGHT NOTICE ONLY This notice does NOT constitute a request for proposal, request for quote, or invitation for bid. No formal solicitation document exists at this time. We are seeking information for market research purposes only. The Government will not pay for any information solicited The VHA Network Contracting Office 6 (NCO 6) is conducting a market survey to find qualified vendors who can provide the following service(s) according to the statement of work below for the Salisbury Department of Veterans Affairs, 1601 Brenner Avenue, Salisbury NC 28144: STATEMENT OF WORK 1. Contract Title. Repair of GE BE9CS Ultrasound Probes 2. Background. All probes are manufactured by General Electric (GE); repair ultrasound probes and return the probes to the end user for continued use for patient care 4. Scope. The following models are all used on the GE Logiq Ultrasound Device: Model Description Problem BE9CS Dual Head Probe S/N 156302YP2 Unresponsive does not function when connected to US device; probe head is damaged BE9CS Dual Head Probe S/N 129017YP0 Unresponsive does not function when connected to US device; probe head is damaged 5. Specific Tasks. All of the probes listed are unresponsive due to defective circuit board located on the probes connection harness assembly. The probes require technical evaluation to see whether they are reparable or require a one-for-one exchange. The problem with each probe is that when the probe is connected to the ultrasound device it does not recognize that the probe is connected to the device and therefore does not produce an image on the ultrasounds monitor. Additionally, the probes have damaged heads which cause poor image quality, which is a safety issue. Normally, once the probe is connected, the machine would recognize the probe and produce an image letting the user know the probe is ready for use. Through normal use and disinfecting process the probes electronic components were potentially damaged and have rendered the probes unresponsive and therefore unserviceable. The desired result we are seeking through this repair service is: Have probe evaluated for damages. Have probe repaired using most economical means. Determine cause of damage and provide documentation stating such. Repair probe in timely manner requesting turnaround of no more than 10 working days. One-for-one exchange of exact model if existing probe is not reparable. Upon completion of repair, perform specification testing to ensure device is performing as intended according to manufacturer s specifications. Interested firms are reminded that in accordance with FAR 4.1201(a) Prospective contractors shall complete electronic annual representations and certifications at the System for Award Management (SAM) as a part of required registration in conjunction with FAR 4.1102(a) that states, Prospective contractors shall be registered in the (SAM) database prior to award of a contract or agreement.
