Page 2 of 2 RFQ Solicitation Amendment Continuing Pages: REFERENCE: VA259-17-Q-0082, A00002, Salt Lake City VAMC Blood Culture and Respiratory Panel Cost per Test Vendor Question and Answer Referencing the Schedule: Line Item Description Estimated Annual Volume Cost per Test/Kit Estimated Total Cost 0001-4001 Blood Culture 200 (100 aerobic and 100 anaerobic) 0002-4002 Supplies (Cartridges & Extraction Trays) 1 0003-4003 Respiratory Panel 5 kits (20 test/kit) 100 tests Question 1: Line item 0002 states 1 Supplies.   I am not sure what I am supposed to fill in there.   Do you want me to just add a per test/kit price next to the Blood Culture and Respiratory line items all inclusive? Answer 1: Vendors should supply information based on the estimated volumes for any costs that are not included in the cost per test.   I know one vendor has the additional cost of verification kits required every 6 months, another system does not.   It s important we consider all the hidden costs required to put out lab results. Question 2: Do you or I email out the Request for Past Performance? Answer 2: The offeror (vendor/contractor) sends out the Past Performance letter and questionnaire in Section D: Attachments to three (3) references. There are instructions in the letter for the reference to return the completed questionnaire to the NCO 19 Contracting Office via email.
