Description

Dear Contractor: This is a request for information (RFI), along with quotations, under solicitation number 612-17-1-129-0028 for the procurement of Overbed Tables w/o Vanity or Storage.   A response to this solicitation is due by 1/5/2016, 1200 EST. The government intends to make a single award firm fixed price Purchase Order but needs to know if there are any vendors that can provide this product/service. Quotations are being requested only to determine if this award can be made at a fair and reasonable price. Purpose: Product/service is needed to replace tables that are broken or damaged around the Martinez Campus. Requirement Description of Overbed Tables w/o Vanity or Storage (see attached Statement of Work and Salient Characteristics) Overbed Tables w/o Vanity or Storage 400 Each (Brand Name or Equal) Installation, Delivery, Project Management 1 Each Delivery will be to the Dept. of Veterans Affairs, Northern California Health Care System; 10535 Hospital Way, Mather, CA 95655. NOTE: RESPONSES TO THIS REQUEST FOR INFORMATION SHALL INCLUDE THE FOLLOWING: 1 Name, Address, Point of Contact, Phone, Email Address 2 - Duns & Cage Information, Socio-Economic Group, annual revenue and employee size, any anticipated teaming arrangements, System for Award Management expiration date, Vetbiz.gov registration and expiration date. 3 - Whether or not your company is a supplier and/or manufacture. 4 - Whether you will be offering an item/service from a Large or Small Business. Please state the name of the business. (if applicable) 5 Whether or not you are an authorized distributor/reseller of this product/service. 6 Whether or not this requirement is on your GSA/FSS/NASA SEWP Schedule and its number or state that your quotation will be Open Market. 7 If you will be offering the brand preferred or an equal. State the equal. 8 A description of similar services offered to the Government and to commercial customers. 9 - Any responses involving teaming agreements/subcontractors should delineate between the work that will be accomplished by the prime and the work accomplished by the teaming partners/subcontractors. (if applicable) The Government will use this information in determining its small business set-aside decision. 10 - Status as a Foreign-owned/foreign-controlled firm and any contemplated use of foreign national employees on this effort. 11 Pricing for this requirement. (Note: No Awards will be made based on pricing submitted. Pricing will be used to determine if the award can be made at a fair and reasonable price and to determine if a solicitation should be issued on GSA E-buy or FBO.gov. If a subsequent solicitation is issued, an award will be made based on the pricing received under the specific solicitation number or using oral solicitation procedures, when applicable.)
