Description
STATEMENT OF WORK (SOW) SCOPE: Contractor shall furnish all supervision, labor, transportation, equipment parts and supplies and expertise necessary to provide preventive bi-annual maintenance, and emergency on-site corrective maintenance visits to all government-owned equipment at the Aleda E. Lutz VA Medical Center as identified on the attached equipment list; see below. This contract will be for one year from date of award. Place of performance shall be at the Aleda E. Lutz VA Medical Center, located at 1500 Weiss Street, Saginaw, MI 48602. QUALIFICATIONS: To be considered eligible for consideration, offerors shall have a field service technician that has been trained by the Original Equipment Manufacturer (OEM) on the types of equipment the technician is being asked to work on. Vendor shall provide documentation of factory certified service/maintenance training on the types of equipment under the terms of this contract. Contractor shall be able to provide 24-hour physical on-site response time to urgently repair critical Blood Gas Instruments. Subcontracting of any ensuing award of this solicitation shall not be allowed. SERVICES TO BE PROVIDED: Contractor shall provide the necessary manpower and supervision to properly execute the maintenance and repair of the equipment listed in Attachment 1; see below. The contractor shall provide OEM parts for any repairs and preventative maintenance of equipment in Attachment 1. Any repair parts with a cost of $100.00 or less shall be purchased by the contractor. The contractor shall supply a written detailed quote to the COR for any parts over the $100.00 cost. The contractor cannot proceed with the additional repair until an authorized purchase order is issued. A Field Service Report (FSR) shall be generated and a copy given to the Aleda E. Lutz VA Medical Center Contracting Officer s Representative (COR) or designee after each repair needed under a normal Aleda E. Lutz VA Medical Center repair Work Order or emergency maintenance. It is preferred that this FSR be provided electronically. COR Nathan Cox, Nathan.Cox@va.gov, 989-497-2500 X13875. Repair: All none emergency repairs shall be performed during normal VA business hours, 8:00am to 4:30pm, Monday thru Friday, except Federal Holidays or any Federal Holiday as authorized by the President of the United States which include: New Year's Day, Martin Luther King, Jr. Birthday, Washington's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day If work must be performed outside of normal business hours ( over-time work), the contractor shall obtain prior approval from the COR. If such work is at the cost of the Aleda E. Lutz VA Medical Center, a purchase order number shall be obtained by the contractor prior to performing any over-time work. If the contractor s telephones are not answered by technical personnel, a technical maintenance representative shall respond by telephone to the VA within 4 hours of a service call from the VA. The contractor shall arrive on-site for repair within 24 hours of placing a service call by the VA. Contractor shall furnish all tools, materials and safety equipment (e.g. service manuals, diagnostic software, etc.) required to maintain the equipment in operating condition. Specific Tasks: Contractor shall perform all manufacturer specified scheduled maintenance as published in the Manufacturer s Service Manual. The Contractor shall have written procedures to be followed and documented evidence that each piece of equipment has been inspected according to those procedures. Scheduled Maintenance shall be performed at the Manufacturer s recommended frequency as published in the Manufacturer s Service Manual during the annual contract period. Contractor shall furnish all supervision, labor, travel, equipment, parts, and supplies to accomplish the preventative or repair maintenance services during the annual contract period. OEM parts shall be used for all maintenance and repair services. Contractor shall provide a copy of the Field Service Report (FSR), either by email or by fax, no later than 6 work days after the completion of the scheduled maintenance or repair service. The Service Report shall contain the following information: Contract purchase order number Date service initiated Date completed Equipment make and unique identification number/co Description of original complaint Problems found Part/quantities installed All work performed fully described COR or designee will verify that the unit is working within Manufacturer s specifications after completion of service or repair. Performance Monitoring: Performance will be monitored by periodic inspections and observation of work. Work will be observed by the COR, or designee, during performance of scheduled maintenance service and inspections and/or during repair service. Equipment operators will be contacted to report on contractor performance when COR, or designee, is not available. The Contractor shall obtain all necessary licenses and/or permits required to perform this work. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to any contractor representative, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by the contractor s fault or negligence.
