Description

The U.S. Department of Veterans Affairs (VA), Network Contract Office (NCO) 18 has a services requirement for Adult Day Health Care (ADHC) for Maricopa County in the State of Arizona. The Adult Day Health Care (ADHC) program is a key component of the Veterans Health Administration (VHA) continuum of extended care services for Veterans at risk of institutionalization under authority 38 U.S. Code § 1720. ADHC is a therapeutically oriented outpatient day program that provides health maintenance, rehabilitative services, socialization, and caregiver support in a congregate setting to enrolled Veterans who meet nursing home level of care. Veterans receiving ADHC are often the frail elderly and functionally impaired. ADHC includes key program elements to address health needs, physical and cognitive function, and the need for social support. The per diem rate is established by the current Medicaid rate for Medicaid approved ADHC s. The NAICS code for this solicitation is 624120 with a small business size standard of $11M. This solicitation will be posted unrestricted. The Government anticipates awarding Firm Fixed Price, Basic Ordering Agreements. Multiple awards may result from this solicitation. The contractor shall perform in accordance with the Performance Work Statement that will be included in the solicitation.    All responsible sources may submit a quotation, which if timely received, shall be considered by this agency. Solicitation Package VA258-17-Q-0022 shall be posted on Fedbizopps on or about January 9, 2017.    No solicitation packages will be mailed.    Responses must be concise and be specifically directed to the requirement referenced above. All offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) at www.sam.gov and complete Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov.
