Description
COMBINED SYNOPSIS/SOLICITATIONS SECONDARY STEAM GENERATOR INSTALLTION SERVICES FOR AN AUTOCLAVE AT THE SYRACUSE VA MEDICAL CENTER (i) This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items and 13.5- Simplified Procedures for Certain Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation is issued as a request for quote (RFQ). Request for Quote will be submitted through fedbizopps.gov on RFQ reference number VA242-17-Q-0085. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-88-1. (iv) This procurement is being issued as a unrestricted open market solicitation. The North American Industry Classification System (NAICS) code is238220, Plumbing, Heating and Air Condition Contractors, with a small business size standard of $20.5 Million. (v) The Contractor shall provide pricing to provide Services below. S ANNUAL TOTAL PRICING FOR SECURITY GUARD SERVICE AGREEMENT Line Item DESCRIPTION QUANTITY UNIT UNIT PRICE TOTAL PRICE 001. STEAM GENERATOR DELIVERY AND INSTALLATION SERVICES FOR SYRACUSE VAMC 1 JB 002 DELIVERY AND INSTALLATION OF HOUSE STEAM/GENERATOR STEAM SWITCH OVER KIT FOR 16 CENTURY PREVAC STERILIZER 1 JB 003 (vi) Description of requirement Statement of Work for Secondary Steam Generator Delivery and Install for Steris Autoclave U.S. Department of Veterans Affairs Syracuse VAMC, New York 1. Scope of Work 1. Vendor shall provide, deliver and install at the Syracuse VAMC, a new steam generator to provide a secondary source of steam for an existing Steris Autoclave, located in the Operating Suite, Floor 2.5 of the Syracuse VA Medical Center. Specifications as follows: Specific requirement Provide and install 30kw 480V 3~ Integral steam generator and house-steam/generator-steam switch over kit for 16"Century Prevac Sterilizer, Serial Number 0331411-14. 50A breaker with wall mounted disconnect and ½ inch hot water connection will be provided by the VA, vendor will connect to these power and water sources. This requirement is to add capability to an existing autoclave. Currently steam is provided to the autoclave via the building s steam distribution. This Statement of Work will provide a secondary source of steam, but not replace the existing source of steam. Warranty Minimum 12 months for parts and labor Work hours will be Monday to Friday evenings, 6pm 2 am or any time on Saturday or Sunday as scheduled with the Contracting Office Representative (COR). Work environment Medical Center infection control procedures must be complied with at all times to include wear of protective clothing and containment of fumes and waste. Additionally, 48-hour notice is required for all burn permits required for any hot work. Installer must be factory trained to work on and modify the Steris 16 Century Prevac Sterilizer. Verification of training will be required prior to commencement of work. Location of work is shown on the attached drawing. Pre-proposal site visit is available upon request. 2. Vender shall completely test the function of the installed steam generator on the autoclave. Test shall be witnessed by VA facility maintenance and SPS personnel. Award shall be made to the quoter whose quotation offers the best value to the government, considering technical capability, past performance, and price. The government shall evaluate information based on the following evaluation criteria: (1) technical capability to meet all of the requirements of the SOW, please provide a maximum 5 page capability statement; (2) Past performance, please provide three references for contract of similar scope and magnitude to be evaluated, The contractors Past Performance shall also be reviewed in CPARs; (3) Price, the government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. (x) The provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items applies to this acquisition. * All firms or individuals submitting a quote shall include a completed copy of this provision, OR have current Representations and Certifications in the System for Award Management (SAM - https://www.sam.gov). (xiii) The Service Contract Act of 1965 does apply to this procurement; the applicable wage determination is attached to this combined synopsis/solicitation (xiv) N/A (xv) This is an open-market total service-disabled veteran-owned small business set-aside combined synopsis/solicitation for Steam Generator Delivery and Installation Services at the Syracuse VA Medical Center as defined herein.    The government intends to award a contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." OR "The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:" The Quoter shall list exception(s) and rationale for the exception(s) Submission shall be received not later than 4:00 PM EST. January 13, 2017. The government shall only accept electronic submissions via email, please send all quotations to Anthony.Murray2@va.gov.
