- Attachment 3 Compensation Plan Attachment 2 Past Performance Questionnaire Attachment 1 Individuals Authorized to Bind the Company Quality Assurance and Surveillance Plan Solicitation Joint Base Lewis McChord Health Contracting Cell (JBLM-HCC) is soliciting for the PERSONAL services of 0.20 full-time equivalent (FTE) Occupational Medicine Physician to provide services within the Occupational Health Clinic at Sierra Army Depot located in Herlong, CA. Primary location of performance is the Occupational Health clinic, but services may be required elsewhere on the installation as directed by the Chief of Preventive Medicine, MAMC. The period of service will begin on or about 1 March 2017 through 29 February 2018, plus four subsequent one-year options ending, 29 February2021, if exercised. * This requirement is set aside for small businesses.
