Description

Solicitation VA244-17-Q-0215 issued 1/10/2017 closes 1/20/2016 is for Dental Lab Services. This requirement is limited to Dental Labs located within 35 miles of the Corporal Michael J. Crescenz Dept. of Veterans Affairs (CMCVAMC) located at 3900 Woodland Ave., Philadelphia, PA 19104. Dental Labs will have to provide American made products. The Dental Labs must allow the CMCVAMC personnel to tour labs prior to award. This requirement is set aside for small businesses. All vendors quoting this requirement must be registered in SAM. Your quote will include your technical capability and dental licenses. You will provide at least three (3) references (past performance information) to include VA Medical Centers and or Hospitals. The past performance information must include Facility, P.O.C name, telephone number, email address, and contract number. All quotes must identify Laboratory name, address, P.O.C. name, telephone number, email address, TAX ID and DUNS number. All quotes must be emailed to the following: Donna.toliver3@va.gov
