SOURCES SOUGHT This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this is to gain knowledge of potential qualified sources and their size relative to NACIS 541612. Responses will be used by the Government to make appropriate acquisition decisions. Responses are not considered responses to any potential solicitation announcement. DRAFT STATEMENT OF WORK (SOW) HOME CARE PHARMACY DISPENSING ACCREDITATION READINESS ASSESSMENT General Objectives and Requirements Title of Project: Home Care Pharmacy Accreditation Readiness Assessment and Consultation Purpose of the Project: Contractor shall provide a review of the facility s compliance with Joint Commission standards specific to home care pharmacy via a mock Joint Commission survey and provide consultation and recommendations to facilitate understanding of the requirements and to bring the facility into compliance for any identified deficiencies. Scope and Methodology: The contractor shall be responsible for conducting a mock Joint Commission home care pharmacy survey of the Michael E. DeBakey VA Medical Center, to include the home care being provided from both the main campus and community based outpatient clinics. The review shall focus on systems and processes associated with the provision of home care pharmacy dispensing services and compliance with Joint Commission standards and requirements. The assessment shall be completed within a 3 day period during the hours of 8:00a.m. 5:00pm. CST. The Contractor shall provide the following Tasks and Associated Deliverables Assessment of compliance with Joint Commission standards related to home care pharmacy services shall include a review of but not limited to: Environment of Care Emergency Management Human Resources Infection Prevention and Control Information Management Leadership Provision of Care, Treatment and Services Performance Improvement Record of Care, Treatment and Services Rights and Responsibilities of Individuals Patient care and system tracer activities applicable to home care pharmacy dispensing. Evaluation of patient records and other information systems that support the delivery of quality patient care as it relates to home care pharmacy dispensing. D. Examination of the organization s Performance Improvement process as it relates to home care pharmacy dispensing services. E. Review of competence assessment process related to the provision of home care pharmacy dispensing services. F. Provide an agenda, outlining each day s activities, a minimum of 2 weeks prior to the readiness assessment G. End-of-day strategic sessions with leadership to discuss the key findings of the day and to provide time for questions. H. Education and consultation with staff, one-on-one and small group, throughout the Readiness Assessment to improve their understanding of The Joint Commission survey processes, quality performance and standards. I. Recommendations for improvement geared toward achieving compliance to be provided throughout the assessment and consultation process. 3. Contractor Qualifications A. The contractor shall be a nationally known and recognized leader in home care pharmacy accreditation readiness consultation with 15 or more years of experience. Consulting staff shall have prior experience as a Joint Commission (JC) surveyor, having participated in numerous surveys or served in a home care pharmacy consultant capacity with extensive experience in guiding organizations through the survey process. The surveyor/consultant must have experience in conducting patient and system tracers, as well as expertise in the Home Care Accreditation process, specific to pharmacy dispensing. The surveyor/consultant must have pertinent knowledge of Occupational Safety and Health Administration (OSHA) regulations, Environment of Care, Infection Control as it relates to pharmacy dispensing. All interested firms shall provide the following information: company name, address, point of contact, phone number, email address, capability statement, business size, DUNS and GSA contract (if any) pursuant to the NAICS code 541612. Responses to this notice shall be e-mailed to Darnell.Rias@va.gov. Telephone responses will not be accepted. Responses shall be received in writing no later than Monday, January 16, 2017 at 10a.m. CST. This notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties shall respond to that solicitation announcement separately from the responses to this announcement. Responses are not a request to be added to a prospective bidders list or to receive a copy of the solicitation.
