Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice.   This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Department of Veterans Affairs, James A. Haley Veterans Hospital has a need to procure, Molecular Diagnostics analyzer systems and reagents capable of running multiplex testing of qualitative detection of Chlamydia Trachomatis, and Neisseria Gonorrhoeae (CT/NG), HPV Genotyping, HCV Viral Loads, HBV Viral Loads, HIV Viral Loads, EGFR, KRAS, BRAF, and qualitative testing of Emergency Use Authorization (EUA) Zika Viruses (all-inclusive Cost per Test Program) response shall include manufacturer plus parts list, along with details on how such item(s) match or exceed the salient characteristics. Solicitation number VA248-17-Q-0128 is issued as a Request for Quotation (RFQ).   This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95, effective 01-13-2017.  This RFQ is unrestricted. The associated North American Industry Classification System (NAICS) Code for this acquisition is 334516, size standard 500 employees. The Contracting Officer plans to award without discussions. The Statement of Work (SOW) can be found in Attachment A. Provisions 52.212-1, Instructions to Offerors Commercial and 52.212-2, Evaluation Commercial Items apply to this acquisition. A Comparative Evaluation of the quotes received shall include the following factors: Price, Technical and Past Performance. Details of the evaluation can be found in the Addendum for FAR 52.212 in Attachment B. Price Schedule and Items Listing use to quote (Base Year and 4 Option Years) in Attachment C. Provision 52.212-3, Offeror Representations and Certifications Commercial items (MAY 2014) applies and is included in Attachment D. Clause 52.212-4, Contract Terms and Conditions Commercial Items applies. Clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders Commercial Items applies and is included in Attachment E. Any questions concerning this solicitation shall be submitted not later than 12:00 PM EST on January 20, 2017, to Trina Berry via email to trina.berry@va.gov. All quotes shall be submitted to Trina Berry via email to trina.berry@va.gov no later than 3:00 PM EST on January 25, 2017.
