Statement of Work Office Modules 2 Statement of Work Office Modules 7 December 2016 Phoenix Veterans Administration Health Care Services 650 E. Indian School Road Phoenix, AZ 85012 Background: Logistics is expanding its staff from 4 to 8 FTE as we move towards total supply support at the Phoenix VA. In order to provide new staff with appropriate work space, it s imperative that we receive office modules. We are requesting one modular for building 22 and two modules for building 26. This equipment does not hold or store patient information. The period of performance will be 30 days after receipt of award. 2.0 TASKS: The vendor, will deliver all necessary equipment to the below shipping address. This will be assembled by our engineering. 3.0 Delivery and Shipping Information: The contractor shall provide the following for shipment and delivery of equipment: All items shall be shipped to the following location: PVAHCS 650 E. Indian School Road Phoenix, AZ 85012-1892 4.0 TRAVEL: N/A 5.0 DELIVERABLES: Deliverable No. Item Quantity Delivery Date 0001 VERSAKING/3" WALL SYSTEM 8'W X 12'L X 8'H / 4-SIDED Three (3) Within 30 Days of receipt of award. 0002 WARRANTY INFORMATION Within 30 Days of receipt of award. 6.0 CONFIDENTIALITY AND NONDISCLOSURE: This item does not hold any patient information therefore a security package is not required. In accordance with Appendix A, Handbook 6500.6 Checklist blocks 7, the C&A requirements do not apply to this requirement, and Security Accreditation Package is not required. Due to the non-technical nature of this requirement, in reference to no. 7 there is no information to be protected. This will be assembled by our engineering. 7.0 GOVERNMENT FURNISHED EQUIPMENT: N/A 8.0 HIPPA Compliance: N/A
