Description
CORPORATE MICHAEL J. CRESCENZ DEPARTMENT OF VETERANS AFFAIRS MEDICAL CENTER LOCATED AT UNIVERISTY AND WOODLAND AVE IN PHILADELPHIA IS SEEKING A QUALIFIED SERVICE DISABLE VETERANS OWNED SMALL BUSINESS (SDVOSB) ARCHITECT/ ENGINEER (A/E) FIRM TO PROVIDE THE INVESTIGATIVE SURVEY AND VERIFICATION OF AS-BUILT CONDITIONS REQUIRED TO FULLY DEVELOP CONSTRUCTION DOCUMENTS FOR THE COMPLETE RENOVATION OF THE DENTAL WING OF BUILDING 1(approximately 6,500 sf). The scope of this project is to provide all the labor, materials, and equipment necessary for developing construction drawings and specification documents for the renovation of the existing Dental Exam Area and associated support areas including but not limited to the following: Installation of new lead lined walls and doors for all exam rooms Replacement of flooring, ceiling tile, and lighting in space Conversion of several storage areas into dental operatory rooms Provide mechanical, electrical and plumbing connections for new, VA furnished dental chairs and wall mounted X-rays in all dental exam rooms. Survey existing conditions and utilizing building as-builts, identify piping in walls and in risers that require replacement or demolition due to abandonment. Provide a study on the existing lead shielding (if any) and determine by said study what lead shielding should be incorporated into the project design drawings. Survey will be conducted by a state certified radiation physicist and shall be reviewed by VA radiation physicist. Provide a cost estimate for the construction costs associated with their final drawings and findings. Work shall be phased to minimize or eliminate service outages. Provide an independent third party life safety review of drawings and specifications The A/E shall collaborate with Medical Center Infection Control Staff to conduct Infection Control Risk Assessment (ICRA) to incorporate the necessary protection as part of construction documents. This A-E Services acquisition is being procured in accordance with the Brooks Act (Public Law 92- 582) and implemented in Federal Acquisition Regulation (FAR) 36. All submissions will be evaluated in accordance with the evaluation criteria. A "short list" of three or more firms deemed most highly rated after initial source selection will be chosen for interviews. The Government will not pay, nor reimburse, any costs associated with responding to this request. The Government is under no obligation to award a contract as a result of this announcement. Firms will be selected based on demonstrated competence and qualifications for the required work. THE NAICS (NORTH AMERICAN INDUSTRY CLASSIFICATION SYSTEM) CODE IS 541310. Before a SDVOSB is proposed as a potential contractor, they must be certified by VETbiz and registered in the System for Award Management (SAM) database. Register via the SAM internet site at www.sam.gov Failure of a proposed SDVOSB to be certified by the CVE at the time the SF 330 is submitted shall result in elimination from further consideration for award.
