- Amendment No. 0001 - The purpose of this amendment is to extend the quotation due date to Wednesday, January 25, 2017. (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested. (ii) Solicitation No. R17PS00006 is being issued as a request for quote (RFQ) for an Indefinitely Delivery / Indefinite Quantity (IDIQ) contract type. (The RFQ and all associated documents may be found by going to the following website: https://www.fedconnect.net/, then clicking on "Search Public Opportunities Only", and then searching by "Reference Number" R17PS00006. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular number 2005-92. (iv) This requirement is a total small business set-aside. The North American Industry Classification System (NAICS) Code for this acquisition is: 561710 - Exterminating and Pest Control Services. The small business size standard for NAICS Code 561710 is $11.0 million. (v) The Bureau of Reclamation, Lower Colorado Regional Office (LCRO), requires an experienced contractor to provide routine, non-routine, and emergency pest control services using Integrated Pest Management (IPM) techniques in accordance with the Performance Work Statement (PWS) and the approved Contractor Work Plan (CWP). The prices from Contract Price Schedule 1 will be used for the first task order. (vi) The Bureau of Reclamation, Lower Colorado Regional Office (LCRO), requires an experienced contractor to provide routine, non-routine, and emergency pest control services using Integrated Pest Management (IPM) techniques in accordance with the Performance Work Statement (PWS) and the approved Contractor Work Plan (CWP). Contractor shall provide all management, licenses/certificates, tools, supplies, equipment labor and transportation to implement a government provided Integrated Pest Management (IPM) plan for pest control services for the LCRO campuses (see the attached Performance Work Statement posted at FedConnect.net) (vii) The ordering period for this Indefinite Delivery / Indefinite Quantity (IDIQ) contract type is estimated to be March 1, 2017 through February 28, 2022. THE FOLLOWING CLAUSES, PROVISIONS, AND ADDENDUMS ARE APPLICABLE TO THIS COMMERCIAL ITEM ACQUISITION: (viii) FAR provision 52.212-1, Instructions to Offerors - Commercial Items is applicable to this RFQ. The following addendum is also applicable to this RFQ. a.Addendum to FAR 52.212-1 - Quote Submission Instructions This addendum replaced the term "offer" with "quote" within the provision 52.212-1. In addition to the requirements of the Instructions to Offerors - Competitive Acquisitions provision of this solicitation, each offeror shall submit a quote in accordance with the instructions contained in this provision. (1) Offer shall complete and submit Attachment 2 - Contract Price Schedule for the Supplies/Services. Pricing must be submitted on Attachment 2. All documents are attached to the RFQ at www.fedconnect.net. (2) Submit the following information: (a)Dun and Bradstreet Number Systems (DUNS): ______________ (b)Contractor E-mail Address: ______________________________ (3) Technical and Past Performance Information Requested: (a)Technical: Offeror shall provide the information below in a written statement to explain their technical capability. A written response is required for each item. 1)Contractor Work Plan (CWP) - Contractor shall prepare and submit a preliminary CWP. The CWP shall specify the schedule, technical approach, and resources required for the planning, execution, and completion of the required services. The CWP shall address continuous monitoring, pest response and removal procedures, record keeping, warranties, education and communication to installation personnel to prevent nuisance pests and disease vectors in and around specified buildings Technical Exhibit (A) - Integrated Pest Management Plan. IPM Plans (Technical Exhibit A) must only be submitted for each building type: Office Space, Warehouse, Industrial, Laboratory Space, and Dive/Boat Shop. The CWP shall include labels, SDS, and the planned pesticide use proposal Technical Exhibit (C) - Contractor Pesticide Use Proposal and comply with all applicable local, state, and Federal regulations. Awardee ¿s preliminary plan submitted with quotation will need to be reviewed and approved by the Sustainability and Environmental Management System (SEMS) committee, as a submittal after award. All cost for revisions and submittals prior to acceptance should be included with your quotation. 2)Offeror must submit all applicable licenses and certifications with technical capability. (b)References: At least three (3) past performance references are requested to be submitted that are most similar to the work of this solicitation that your company has performed in the past three years. 1)Offerors shall have their references complete and submit the attached Past Performance Questionnaire (PPQ) in attachment at FedConnect.net (Attachment 4 - PPQ). PPQ must be submitted directly from your reference to the contracting office via email or fax to Leslie Dieguez at ldieguez@usbr.gov or fax to (702) 293-8332. 2)It is the Offerors responsibility to ensure that at least three (3) different references submit past performance questionnaires prior to the quotation due date. PPQs provided directly from the offeror will not be considered. NOTE: An offeror without relevant past performance, or for whom information on past performance is not available, will be given a Neutral rating, and will not be evaluated favorably or unfavorably. This rating is neither negative nor positive. Neutral is merely indicative of a lack of prior performance. Government may also use past performance information obtained from other than the sources identified by the offeror. Information obtained from the Past Performance Information Retrieval System (PPIRS) database located at http//www.ppirs.gov is one of the sources that will be utilized. PPIRS functions as the central warehouse for performance assessment reports received from several Federal performance information collection systems and is sponsored by the DOD E-Business Office and administered by the Naval Sea Logistics Center Detachment Portsmouth. The Government will also use information obtained from The Federal Awardee Performance and Integrity Information System (FAPIIS) http://www.fai.gov/FAPIIS/trailer/module.htm will be utilized as well. (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.
