Description
This is a Sources Sought Notice. This is NOT a solicitation for proposals, proposal abstracts or quotations. The purpose of this notice is to obtain information regarding the availability and capability of qualified sources. The NAICS code is 339112 and the size standard is 500 employees. The Joint Base Lewis McChord Health Contracting Cell (JBLM), Madigan Army Medical Center, Department of Surgery (MAMC) has a Brand Name Only requirement for Surgical Implants such as Orthloc Poly axial Calcaneal Fracture Plate, Graft Jacket Allograft or Graft Jacket Biologic; Tornier AEQ reverse locking screws; Lisf ranc Screws Implants; Allopure Allograft Bone Wedges; DBM Demineralized Bone Matrix; Subtalar Implants and Shoulder Fracture Plate, Screws, X-ream for Femoral Neck Fracture Pinning; Screws, X-ream for Femoral Neck Fracture Pinning; Anterolateral Plates for Left Tibiotalar Arthrodesis;Bone Graft Injectable; CF Amniotic Barrier Membranes; Power Mix Injectable Stimulus;Shoulder System;Jones Fracture System; Compression Plating System; Comprehensive Cannulated Screw System; Tendon Tensioning and Superior Fixation Strength with a Radiolucent Screw; Titanium Suture Anchor; Hammertoe Fixation System; Hammertoe Implant; Fusion Bolts and Beams System; Elbow Plating System; Poly axial Calcaneal Fracture Plate and Poly axial Calcaneal Fracture Plate products from the Wright Medical product lines. The Surgeon has determined that this product line will best meet the patients' specific medical needs/requirements. ALL PRODUCTS MUST BE FDA APPROVED and meet the surgeon's and the patient's specific medical needs/requirements. Wright Medical Surgical Implants will be used for patient care within JBLM medical treatment facilities which is Madigan Army Medical Center (MAMC) and Bassett Army Community Hospital (BACH), Fort Wainwright AK And all items must be compatible with existing Wright Medical implants. The Joint Base Lewis McChord Health Contracting Cell (JBLM), intends to acquire a commercial item using FAR Part 12. This procurement is being conducted under FAR Part 13 Simplified Acquisition Procedures. Interested organizations may submit their capabilities and qualifications to perform the effort in writing no later than 23 Jan 2017 at 07:00 a.m Pacific Standard Time. A determination by the government on how to compete this proposed effort, based upon responses to this notice, is solely within the discretion of the government. Any vendor that believes it can provide the stated supplies/equipment can provide supporting documents demonstrating their products capability and specifications. ALL PRODUCTS MUST BE FDA APPROVED, AND MEET THE SURGEON/PATIENT REQIREMENTS. Responses to this notice should include company name, address, telephone number, point of contact (POC), and brochures which will allow the Government to understand your offerings and options. All questions shall be submitted via email to Purchasing Agent; Ms. Angela Dexter at angela.a.dexter.civ@mail.mil. Telephone responses will not be accepted. All vendors providing to the Federal Government are required to be registered in the System for Award Management (SAM) registry at with NAICS code 339112. The SAM internet web site is http://www.sam/gov.
