- Attachment J-1 Floor Layouts. Solicitation. (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is LCNLS17R0004 and issued as a Request for Proposal (RFP). (iii) This solicitation document incorporates all mandatory commercial item provisions and clauses currently in effect in the latest Federal Acquisition Circular (FAC) 2005-95. (iv) The NAICS code is 484210. There are no set-asides. (v) Offerors shall provide pricing for all the deliverables in Section B of the attached Statement of Work. (vi) The National Library for the Blind and Physically Handicapped (NLS) occupies a building which is a two story building occupied by approximately 115 Library employees, a number of whom are blind and/or physically handicapped. The NLS building will undergo renovation which will require the partial relocation of some staff to offices in an alternate location in downtown Washington, D.C., and the sequential relocation of the remaining staff in the NLS building. The NLS seeks a contractor to provide relocation and packing services during the phased renovation project. The attached Statement of Work, contains the requirements to be satisfied by the successful offeror awarded a contract as a result of this solicitation. (vii) The Library anticipates a single-award Firm Fixed-Price (FFP) contract to obtain the services. See Section C of the solicitation for anticipated contract period of performance. (viii) The provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. Refer to attached "Terms and Conditions" for additional proposal submission instructions. (ix) The provision 52.212-2, Evaluation-Commercial Items applies to this acquisition. Refer to attached "Terms and Conditions" for evaluation factors. (x) Offerors are advised to include a completed copy of the provision at 52.212-3, Offeror representations and Certifications-Commercial Items with their offers. (xi) The clause at 52.212-4, Contract Terms and Conditions-Commercial items, as modified by the Library, applies to this acquisition. Offerors shall refer to clause 52.212-4 listed in attached "Terms and Conditions" for additional clauses applicable to this acquisition. (xii) The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial items as modified by the Library, applies to this acquisition. Offerors shall refer to clause 52.212-5 listed in attached "Terms and Conditions" for additional clauses applicable to this acquisition. (xiii) Additional contract requirements, terms and conditions are determined by the Contracting Officer to be necessary for this acquisition and consistent with customary commercial practices. The following attachments are included and are hereby incorporated into this solicitation: (1) SOW, and (2) Terms and Conditions. (xiv) The Defense Priorities and Allocations System (DPAS) does not apply to this requirement. (xv) Questions concerning any areas which, in the offeror's opinion, require clarification or correction must be submitted in writing. See Section L.1.B of the solicitation. Answers to questions of a substantive nature will be provided to all offerors. (xvi) Proposals are due by the time and date indicated on Federal Business Opportunities. There are no exceptions to the time and date the proposals are due. Any proposal submitted after this time will not be considered. See Section L.1.A of the solicitation. It is the offerors responsibility to ensure that the proposals are submitted timely. (xvii) Proposals are due as shown in Bock 8 on page 1 of the solicitation.
