- COMBINED SYNOPSIS/SOLICITATION Medivators-Advantage Plus Endoscope Reprocessor at Michael E. DeBakey Veterans Affairs Medical Center in Houston, Texas (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number: VA256-17-Q-0238. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-87. (IV) This solicitation is being issued as a Total Service Disabled Veteran Owned Small Business Set-Aside. The associated NAICS code is 339112; the small business size standard is 1000 employees. (V) This combined solicitation/synopsis is for purchase of the following commercial services: Contractor shall furnish all labor, equipment, skilled and certified personnel to provide Medivators-Advantage Plus Endoscope Reprocessor at the Michael E. DeBakey Veterans Affairs Medical Center main campus located at 2002 Holcombe Blvd, Building 100. Houston, Texas 77030. (VI) Description of requirements is as follows: Contractor shall furnish Medivators-Advantage Plus Endoscope Reprocessor that meets the following description: Item Number Item Name Qty Price Unit ADV-1007 Advantage Plus SS with Compressor 3 ea. ADV-1062 ADV, Active Vapor Mgt 3 ea. FV020012 ASSY, Hook-up, OLY 6 ea. SE011072 Install Advantage BD 3 ea. SE0172021 ADV Plus Operator Train 3 ea. (b) The place of acceptance will be: Michael E. DeBakey VAMC, 2002 Holcombe Blvd, Houston, Texas 77030 Period of performance The period of performance of this contract is tentatively set for a 30 ARO not to exceed 09/30/2017. Quotes are required to be received in the contracting office no later than 11:30 A.M. CST on January 27, 2016. All quotes must be emailed to the attention of Kever L. Swafford.
