- NDAA 2016 NDAA 2008 THIS IS NOT A SOLICITATION ANNOUNCEMENT. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their business size classification (HUBZone, 8(a), Small Business, Small Disadvantage, Small Veteran Owned Business or Service Disabled Veteran Owned Small Business) relative to NAICS Code: 624190- Other Individual and Family Services (Size Standard of $11 Million). Responses to this synopsis will be utilized by the Government to determine the appropriate procurement methodology. After review of the responses to this sources sought synopsis, a solicitation announcement may be published to the general public on the Federal Business Opportunities website at www.fbo.gov. Responses to this sources sought synopsis shall not be considered as a response to a solicitation. All interested offerors will respond as an interested party to this sources sought. The NH National Guard is seeking sources for a potential contract to provide care coordination and referral services to members of the US Armed Forces as defined in 10 USC 101(a) (4.) and (c) and DODI 1342.28 and their Families as defined in 37 USC 401 within the state of New Hampshire. Purpose: The goal of the program is to enhance the retention, readiness and reintegration of Service Members and their Families. The beneficiaries of the Care Coordination Program are all National Guardsmen, Reservists, and Active Duty Service Members and their Families who are geographically separated from military installations and live or work in the State of New Hampshire. For data collection and historical reporting, this population is divided into Service Members and Families as defined by the NDAA 2008 (PL 110-181), and further clarified in NDAA 2016 (PL 114-92) and 37 USC 401. The Care Coordination Program will meet its purpose through two primary objectives: Care Coordination and Outreach. Care Coordination: Care Coordination is personalized and substantive services provided to Service Members and their Families. There are four types of Care Coordination: Direct care, Indirect care, Clinical Care and Personal Financial Care. Direct, Indirect and Personal Financial care are the priority, though Clinical care is authorized during emergency situations or when significant barriers to entry to outside agencies exist. Direct Care: This is provided to clients in a face-to-face meeting and can include intake, referrals, assessment of needs, and development of care plans, coordinating care, and follow-up. Care Coordination involves identifying unmet needs, and developing plans to meet those needs in a timely manner. Care Coordination is always intended to supplement, not supplant, services available from federal, state and community-based organizations; Direct Care is broadly defined as face to face Care Coordination and referrals that maximize participation inside of those federal, state, and community based organizations. The ultimate goal is the successful completion of care plans. Indirect Care: This provides the same level of care and services described under direct care but is provided through means other than a face-to-face meeting such as telephone, text, email, etc. The ultimate goal is the successful completion of care plans. Clinical Care: This is care by a licensed clinical social worker during emergency situations or when significant barriers to entry to outside agencies exist. Clinical Care can be considered those services which would otherwise be provided by outside agencies at the federal, state, and local level, but barriers such as timeliness and availability affect access. Personal Financial Care: This is provided to clients in either a face-to-face meeting or by telephone, email, text and other forms of communication and includes referrals, assessment of needs, development of a financial plan, education and counseling as needed and follow-up. The ultimate goal is the successful completion of the financial plans. Outreach: Outreach is divided into Community Capacity Building and Outreach to military units, Service Members and their Families. Community Capacity Building: The goal is to develop relationships with local providers in the state who can provide needed services to Service Members and Families, and to leverage these local providers in the execution of the Care Plans. These activities involve regular attendance at various committees, education and outreach to providers, and routine liaison with providers throughout the state. These activities both create and coordinate military/civilian relationships, thus creating a stronger safety net for our military. Outreach to military units, Service Members, and their Families: The goal of these activities is to increase awareness of the Care Coordination Program and resources available to Members of the Armed Forces and their Families. Activities include but are not limited to outreach education at unit drills, family days, organization days, or at other events when invited by a unit; outreach at veteran fairs and events throughout the state; and will include a plan to proactively market to Families and active duty individuals geographically dispersed throughout the state. The Government intends to issue a FIRM FIXED PRICE three year contract to include a base year and two (2) additional one year option periods; which is to begin on or around 22 Aug 2017. If interested and capable of providing the required services please provide the requested information: (1) Is your business small? (2) If small, does your company qualify as a small emerging business, or small disadvantage business? (3) If disadvantaged, specify under which disadvantaged group and if your company is certified under Section 8(a) of the Small Business Act? (4) Is your company a certified, HUBZone company? (5) Is your company a woman-woman-owned business? (6) Is your company a certified Service-Disabled Veteran Owned? (7) Is your company a Veteran Owned Small Business? Small business concerns that are considering participation in this potential solicitation process under any Small Business Set-aside shall do so in accordance with the Small Business Act. Responses to this notice shall be e-mailed to the attention of: isao.hattori.mail.mil. Telephone responses will not be accepted. Responses must be received no later than 4:30 p.m. on 7 February 2017. This notice is to assist in determining the availability of capable sources via the commercial industry only. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties shall respond that solicitation announcement separately from the response to this announcement.
