Description
52.212-5 Statement of Work This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 "Streamlined Procedures for Evaluation and Solicitation for Commercial Items," as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation document will not be issued. Submit offers on RFQ-RML-E-1821614. This acquisition is a Request for Quotes (RFQ). The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-93-1 1/1/2017. The associated North American Industry Classification System (NAICS) code for this procurement is 518210 with a small business size standard of ($32.5M). This requirement is not set aside for small business. The National Institute of Allergy and Infectious Diseases (NIAID), has a need for contractor services for data analysis, review and data entry in the Primary Immune Deficiency Treatment Consortium Database System (PIDTC); with a period of performance of 02/15/2017 to 02/14/2018; as well as option year one period of performance 02/15/2018 to 02/14/2019; and option year two period of performance 02/15/2019 to 02/14/2020. Estimate of 1000 hours per year. Quotes shall be expressed in price per hour. See details in attached SOW. • Contractor must be located in the surrounding areas of Bethesda, MD area as site visits and meetings are a requirement. • Contractor must be able to obtain access to the NIH Bethesda, MD campus and NIH systems. Official access of systems and locations must be acquired after award and sustained throughout the entirety of the contract, including option years. • Contractor must be able to obtain all required security training to possess and use a government issued laptop, and access secure government files. This training must be kept up to date for the entirety of the award, including option years. • Contractor must have been previously employed in an occupation in which they have firsthand experience with Severe Combined Immunodeficiency (SCID), Hematopoietic Cell Transplantation (HCT), Gene Therapy (GT) and Enzyme Replacement Therapy (ERT), leaky SCID, Omenn Syndrome. Reticular Dysgenesis and Chronic Granulomatous Disease in order to assess diagnoses, medications, and adverse reactions to accurately compete the required PIDTC forms. Contractor must have the knowledge of the medical terms, diagnosis, etc. that relate to the aforementioned conditions. • A detailed curricula vitae (CV) must be provided for the individual responsible for completing the service. Place of Performance: NIH/NIAID Bldg 10 10 Center Drive Bethesda, MD 20814 United States Award will be based on the capability of the item offered to meet the above stated salient characteristics, price, past performance, delivery, warranty, and the best value to the government. Evaluation of options will not obligate the Government to exercise the option(s). *The applicable subparagraphs of FAR52.212-5 are included in the attachment to this posting.
