Sources Sought Notice: DESCRIPTION: This is a sources sought to determine the availability of potential sources having the skills and capabilities necessary to provide the SPECIFIED REQUIREMENT. All interested vendors are invited to provide information to contribute to this market survey/sources sought including commercial market information. THIS IS NOT A SOLICITATION ANNOUNCEMENT. This is a sources sought synopsis only. Questions shall be submitted by email to Jennifer.adams7@va.gov. Provide only the requested information below. The purpose of this sources sought is to gain knowledge of potential qualified sources and their size classifications (Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), Hub zone, 8(a), small, small disadvantaged, woman owned small business, FSS/GSA contract schedule holders or large business) relative to NAICS 811219 Other Electronic & Precision Equipment Repair and Maintenance. Responses to this sources sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published on the GSA or FBO website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. SPECIFIED REQUIREMENT: Network Contracting Office 16, Jackson, MS is seeking sources for a potential contractor to perform the following type of service for Preventative Maintenance & Repair Service Agreement of Cepheid Equipment (Model # GX16S1Y, Serial # 800125) Services. Reference attached SOW listed below. STATEMENT OF WORK 1. Contracting Officer s Representative (COR). Name: Section: Biomedical Engineering Address: 1500 E Woodrow Wilson, Jackson, MS 39216 Phone Number: Fax Number: E-Mail Address: 2. Contract Title. Cepheid GeneXpert Service Contract 3. Background. G.V. (Sonny) Montgomery VA Medical Center is requesting the maintenance and repair service from Cepheid for the GeneXpert laboratory equipment in Jackson, MS. This contract shall be for base plus four (4) option years. 4. Supplies or Services and Price/Cost The contractor shall furnish all necessary labor, parts, transportation and equipment to provide preventive maintenance and service agreement for 1 Genexpert, sixteen (16-16) modules within G.V. (Sonny) Montgomery VA Medical Center in accordance with all terms, conditions, provisions, schedules and specifications of this contract for a period of March 1, 2017 through September 30,2017 This contract shall be handled according to the Federal Acquisition Regulation (FAR) Part 12 for Commercial Items and Services and the contract clauses contained within. Payment shall be made monthly in arrears upon receipt of a proper invoice at the G.V. (Sonny) Montgomery VA Medical Center. Total POP: March 1, 2017__ QTY UNIT PRICE 1. Genexpert, sixteen module, DX 7 MTH $13,005.00 16-16 s/n 800125 VAMC Jackson Total Cost: $13,005.00 4. Scope. The equipment covered under this service agreement is listed in the table below and located at the G.V. (Sonny) Montgomery VA Medical Center. Service shall consist of preventive optical and thermal calibrations and replacing parts and failed modules to maintain the equipment within original specifications. The service maintenance agreement shall include 24x7 phone support; onsite 5x8 hours (8AM-4PM); annual XpertCheck; cover parts labor, and travel; and any and all software upgrades. Equipment Name Site Number Model #: Serial #: EE #: GeneXpert 586 GX16S1Y 800125 N/A The Contractor shall furnish all necessary labor, parts, transportation and equipment to provide preventative maintenance and service to Government-owned equipment listed in B.1.2 above. Laboratory point of contact (POC) information and equipment location is provided below for the period of March1, 2017 through September 30, 2017. VAMC Jackson Lab Manager, P&LMS (113) 1500 E. Woodrow Wilson Jackson, MS 39216 (601) 362-4471 The Contractor shall provide the COTR with the proper procedure for an emergency call to be handled, including the service representative's name and telephone number. This information shall be supplied to each participating medical center for their use as needed. 5. Specific Tasks. The equipment covered under this service agreement is listed in the table in Section 4: Scope. Service shall include automatic exposure, battery replacement, and tube coverage for the equipment listed. Expectations of the contractor are detailed below. The Contractor shall contact and coordinate with the local VA representative a mutually agreed upon time to complete the service or maintenance. All work shall be accomplished during normal duty hours 8:00 am to 4:30 pm, Monday through Friday, excluding federal holidays. The following are federal holidays: New Year s Day Labor Day Martin Luther King Jr. s Birthday Columbus Day President s Day Veteran s Day Memorial Day Thanksgiving Day Independence Day Christmas Day 6. Performance Monitoring Vendor performance shall be monitored by Biomedical Engineering, the COR, and using service to ensure conformance to stated contract requirements and expectations. Work completed shall be reported to the COR to ensure that contractual conditions are met. 7. Security Requirements The C&A requirements do not apply and a Security Accreditation Package is not required. 8. Place of Performance. Service and maintenance work shall be completed on site at the G.V. (Sonny) Montgomery VA Medical Center Jackson VA Medical Center is a smoke free facility. Smoking is prohibited inside any building except in a designate area on the facility grounds. Smokers are allowed to smoke outside the 25 foot boundaries from every entrance/exit. Violators shall be removed from the premises permanently Contract clause for Information and Information Technology Security: a. Due to the threat of data breach, compromise or loss of information that resides on either VA-owned or contractor-owned systems, and to comply with Federal laws and regulations, VA has developed an Information and Information Technology Security clause to be used when VA sensitive information is accessed, used, stored, generated, transmitted, or exchanged by and between VA and a contractor, subcontractor or a third party in any format (e.g., paper, microfiche, electronic or magnetic portable media). b. In solicitations and contracts where VA Sensitive Information or Information Technology will be accessed or utilized, the contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract. The contractor shall request logical (technical) or physical access to VA information and VA information systems for their employees, Subcontractors, and affiliates only to the extent necessary to perform the services specified in the contract, agreement, or task order. All Contractors, Subcontractors, and third-party servicers and associates working with VA information are subject to the same investigative requirements as those of VA appointees or employees who have access to the same types of information. The level and process of background security investigations for Contractors must be in accordance with VA Directive and Handbook 0710, Personnel Suitability and Security Program. The Office for Operations, Security, and Preparedness is responsible for these policies and procedures. 9. Period of Performance. Vendor shall provide service for the period March 1, 2017 - September 30, 2017, at G.V. (Sonny) Montgomery VA Medical Center. REQUESTED INFORMATION: (1) STATEMENT OF CABABILITY: Submit a brief description, five (5) pages or less, that demonstrates how your company has the supervision, facilities, labor and experience to perform the scheduled and unscheduled repairs and preventative maintenance of the equipment described above. Include past experience in performing these services to the VA, other Government (Federal or State) agency, or for a private facility. Please specify your availability to start date and your address. (2) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub-zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm. (h) State whether your firm is registered with the System for Award Management (SAM) at https://www.sam.gov/portal/public/SAM/ and/or the VetBiz Registry at http://vip.vetbiz.gov/. (i) NOTE that any future solicitation could only be awarded to a contractor who is registered in SAM and to receive award based on VOSB or SDVOSB status you must be registered in the VetBiz Registry. Responses shall be submitted to VA256-17-N- 0241 no later than 12:00pm CST, Thursday, February 2, 2016. Fax or Telephone calls will not be accepted. Your response should include both the STATEMENT OF CABABILITY and BUSINESS SIZE AND SOCIO-ECONOMIC STATUS information as explained above.
