- PWS RFQ-Worksheet Combined Synopsis - Solicitation For Commercial Items (IAW FAR 12.603 (c)) (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. FAR subpart 13.5, are applicable. This announcement constitutes the only solicitation. Proposals are requested and a written solicitation will not be issued. (ii) This solicitation/synopsis reference number is F1M3E36357AW01 and is being issued as a Request for Quote (RFQ). The RFQ format is attached to this combined synopsis/solicitation and shall be used to submit pricing. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-93, effective 01 January 2017. (iv) This acquisition is 100% set-aside for Small Business. The associated NAICS code is 561710 with a $11M size standard. (v) Contractors shall submit a proposal in reference to the attached Performance Work Statement (PWS). All responsible sources may submit a quotation, which shall be considered. (vi) The contractor will be responsible for providing all materials, labor, equipment, tools, supervision, and incidentals to perform Pest Control Services on Joint Base Charleston Weapons Station, Short Stay Recreational Facility and the old Naval Shipyard, South Carolina (see attached Performance Work Statement). (vii) FOB Destination - The place of performance is Joint Base Charleston - Weapons Station, Short Stay Recreation Facility, and the old Naval Shipyard, South Carolina. The period of performance is as follows: Six month Base Year: (1 April 2017- 30 Sep 2017; and four 1 year option periods: 1st Option (1 Oct 2017 - 30 Sep 2018); 2nd Option (1 Oct 2018 - 30 Sep 2019); 3rd Option (1 Oct 2019 - 30 Sep 2020); 4th Option (1 Oct 2020 - 30 Sep 2021). 6 month extension 1 Oct 2021 through 31 March 2022. ***The extension would only be used in the event of a protest or other unforeseen events that result in the delay of a follow-on award action. (viii) The provision at FAR 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition and is amended as follows: A site visit will be conducted on 2 February 2017 at 10:00 A.M. (EST) Site visit attendees will meet at the Redbank Club located at Bldg 942, Red Bank Rd, Naval Weapons Station, Joint Base Charleston, SC 29420. Special arrangements will be required to gain access to JB Charleston, SC. Offerors are cautioned that Joint Base Charleston has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass prior to entrance. SOME DELAY SHOULD BE ANTICIPATED. Offerors should allow sufficient time to obtain a visitor pass (requiring valid driver's license, Social Security Card, vehicle registration, and current proof of insurance). Proposals must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The response shall consist of two (2) separate parts; Part I - Price Proposal, Part II- Past Performance. Only references for same or similar type contracts are desired. Provide no more than three references of "Recent" and "Relevant" projects. Recent" is defined as projects within the past three years. "Relevant" is defined as; performance effort involved essentially the same magnitude of effort and complexity this RFQ requires. This includes but is not limited to the control the following types of pests: nuisance, structural, lawn and turf, ornamental, and medical arthropod and vertebrate pests. The evaluation of past performance information will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the instant acquisition. Furnish the following information for each contract referenced: (i) Company/Division name (ii) Service (iii)Contracting Agency/Customer (iv)Contract Number and Contract Type (e.g. Firm Fixed-Price, Time & Materials, etc.) (v) Contract Dollar Value (Initial, final and an explanation of the difference) (vi)Period of Performance (vii) Verified, up-to-date name, address, FAX and telephone number (viii) Contracting Officer Representative's name, address, telephone & fax numbers (ix)Brief summary of the requirements (x) Comments regarding compliance with contract terms and conditions (xi)Comments regarding any known performance deemed unacceptable to the customer, or not in accordance with the contract terms and conditions (ix) FAR 52.212-2, Evaluation - Commercial Items, applies to this acquisition and is amended as follows: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. 1. Price - The Government will evaluate the total price of the offer for award purposes. 2. Past Performance. The basis of the award will be made on Price and Past Performance with Performance being significantly more important than price. If the lowest priced evaluated responsible offer is judged to have a "Substantial Confidence" performance assessment rating, that offer represents the best value for the Government and the evaluation process stops at this point. Award shall be made without further consideration of any other offers. If the lowest priced offeror is not judged to have a "Substantial Confidence" Performance Assessment Rating, the next lowest priced offer will be evaluated and the process will continue (in order by price) until an offeror is judged to have a "Substantial Confidence" performance assessment or until all offerors are evaluated. The Source Selection Authority shall then make an integrated assessment best value award decision. Offerors will provide no more than three references of "Recent" and "Relevant" projects. Recent" is defined as projects within the past three years. "Relevant" is defined as; performance effort involved essentially the same magnitude of effort and complexity this RFQ requires. The government reserves the right to award a contract to a higher priced offeror if the lower priced offeror(s) has a lower performance confidence assessment. (x) All offerors shall include a completed copy of FAR 52.212-3, Offeror Representation and Certifications--Commercial Item or complete electronic annual representations and certifications at http//orca.bpn.gov. (xi) The clause at 52.212-4 Contract Terms and Condition-Commercial Items, applies to this acquisition and is amended as follows: BUSINESS LICENSE TAX - SOUTH CAROLINA In accordance with Contract Clause entitled, "Federal, State and Local Taxes" dated APR 2003/FAR 52.229-3 incorporated by reference, be advised that the Cities of North Charleston, South Carolina and/or Goose Creek South Carolina imposes a business license tax on all contract work within the cities of North Charleston, South Carolina and/or Goose Creek South Carolina. The cities interpret this to include contractors performing work for the United States government within the limits of North Charleston and/or Goose Creek. Joint Base Charleston - Air Base is included within the City of North Charleston, South Carolina and Joint Base Charleston - Weapons Station is included within the city limits of Goose Creek. The cost of the license is proportional to the dollar amount of work being executed.
