We are seeking information for market research purposes only. The Government will not pay for any information solicited The VHA Network Contracting Office 6 (NCO 6) is conducting a market survey to find qualified vendors who can provide the following service(s) according to the statement of work /description of item(s) below for the Fayetteville Department of Veterans Affairs Medical Center (VAMC), 2300 Ramsey Street, Fayetteville NC 28301: STATEMENT OF WORK/ DESCRIPTION OF ITEMS VAMC FAYETTEVILLE NC Multi-Specialty Table/ Ultra Scan Table A. GENERAL GUIDANCE 1. Description of Multi-Specialty Table/ Ultra Scan Table: UltraScan Premier Table Foot Drop Section - Manually Adjustable/Removable, Fowler- 0 -90 degrees - Electrically Adjustable Height - Electrically Adjustable, 24.5 - 34.5 inches Trendelenburg +15 degrees/rev. Trendelenburg -25 degrees - Electrically Adjustable 500 lb. distributed load capacity 500 lb. distributed lift capacity Self-storing adjustable stirrups Built-in self-storing foot board 5 inch, 2-Way Locking Casters 1 year warranty (See Expanded Specifications/Warranty) Protected by Silverguard antibacterial technology 2. Scope of Work: The contractor shall provide all labor, supervision and all other resources required to deliver the requirements stated herein, except as may otherwise be specified in this statement of work. 3. Background: The overall design of the ultrasound exam table should incorporate features that allow for multiple adjustments, ease of mobility and accessible brakes that can be quickly activated. Features should allow the sonographer to quickly position the table within the room to achieve the most comfortable work position. Features should not interfere with positioning the patient close to the user or positioning the unit close to the table. The table should be designed so that the sonographer can use it from either a standing or sitting position and must be height adjustable. Specific designs should be in place to allow the user to adapt the table for many different exams. 4. Performance Period: Vendor shall deliver fully functional tables at the request of the Medical Center at 2300 Ramsey Street, Fayetteville, NC. In consideration of VA s agreement to perform the terms in the purchase agreement, the vendor will assign a representative to deliver/set-up the tables at the designated site or deliver tables fully assembled. B. CONTRACT AWARD Vendor shall be paid for equipment only when equipment has been received and inspected to be in good working order. C. GENERAL REQUIREMENTS Equipment delivered shall include a multi-specialty table, side rails and all components. In consideration of VA s agreement to perform the terms assigned in the purchase agreement, the vendor will assign a representative to deliver/set-up the tables at the designated site or deliver the tables fully assembled. D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Vendor will provide support for one (1) year following delivery/set-up at no additional cost to VA. Vendor will provide support, Monday through Friday, from 7:00 am until 5:00 pm, Eastern Time, excluding holidays. This support may be rendered by voice or electronic mail. E. EVALUATED OPTIONAL FEATURES On-site service calls are not included, but will be made available to VA at additional cost. VA will not modify any equipment without prior permission of the vendor. F. SCHEDULE FOR DELIVERABLES N/A G. CHANGES TO THE STATEMENT OF WORK The process for making changes to the SOW is presented in this section. Making changes by any other means may require ratification of an unauthorized action requiring the approval of the Chief, VISN 6 CAS Office. H. REPORTING REQUIREMENTS N/A I. TRAVEL N/A J. GOVERNMENT RESPONSIBILITIES The vendor will agree to the terms and conditions contained in this statement of work regarding the purchase of equipment and service. Any change in any of the terms and conditions of this agreement must be in writing. K. CONTRACTOR EXPERIENCE REQUIREMENTS Qualifications of Key Personnel The vendor shall identify, by name, the Customer Service Specialist, Sales Representative and Clinical Application Specialist who will work under this task order at the time the work is being negotiated. If a key person becomes unavailable, the proposed substitutions of personnel shall be made only as approved directed by the Contracting Officer and the COTR. The government will not dictate specific experience and education requirements of the employees initially proposed to perform the work stated herein. L. CONFIDENTIALITY AND NONDISCLOSURE Standard mandatory language for all task orders. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS New contractor personnel security requirements, which facilitate the security programs for computer systems and automated information systems implemented by the Office of Security and Law Enforcement and Veterans Health Administration. Interested firms are reminded that in accordance with FAR 4.1201(a) Prospective contractors shall complete electronic annual representations and certifications at the System for Award Management (SAM) as a part of required registration in conjunction with FAR 4.1102(a) that states, Prospective contractors shall be registered in the (SAM) database prior to award of a contract or agreement. Therefore, due to time constraints, vendors will be required to be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award.
