Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 "Streamlined Procedures for Evaluation and Solicitation for Commercial Items", as supplemented with additional information included on this notice. This announcement constitutes the only solicitation; offers are being requested, and a written solicitation document will not be issued. The Government intends to issue a purchase order in accordance with FAR Part 13 for using Simplified Acquisition Procedures. The solicitation number is VA247-17-Q-0205. This solicitation is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular FAC 2005-70 effective September 30, 2013. The associated North American Industrial Classification System (NAICS) code for this procurement is 337215 and Product Service Code (PSC) 7125. This combined/synopsis solicitation is set aside for Service Disabled Veteran Owned Small Businesses (SDVOSB s) in accordance with VAAR 852.219-10. Only VA verified SDVOSB s in the VIP at the time of contract award will be considered. All non-VIP verified firms will be considered non-responsive and ineligible for award. The acquisition will be made pursuant to the authority in FAR 13 to use simplified procedures for commercial items. The Department of Veterans Affairs, Birmingham VA Medical Center has a requirement for Statement of Work Birmingham VA Medical Center Space Saver High Density Storage System (521-17-1-5129-0028) SCOPE OR WORK: The Birmingham VA Medical Center (BVAMC) located at 700 South 19th Street, Birmingham, Alabama 35233, requires a Space Saver High Density Storage System for patient health related items. The successful bidder shall provide specified products, delivery and installation at the jobsite. Specifics are: We require one (1) Spacesaver High Density Storage System (or equivalent) with anti-tip, continuous (no Splice Joints) fully grouted rail, providing support for 1,000 pounds per foot. Carriage with shelf retainer lip. Platform to be used with fixed units to provide harmonious system. Carriage and Platform to be unitized, jig welded components joined with concealed moment connections, maintaining 1,000 pounds per foot support. Laminated End Panels to be provided on exposed ends. Carriage panel to support ergonomic three-spoke turn handle with integrated safety color-coded aisle lock and exterior chain tensioner. Each panel to have Label Holder for each aisle. Raised floor rated for 900 pounds per sq. ft., to be placed between rails covered with VCT, selected from standards. Carriage shall move on the rails smoothly without binding and free of yaw. Operation to be one pound of effort when turn handle moves 4,000 pounds of weight and travels 5.25 inches, per revolution. Shelves to be Key-slot design free of exposed holes, closed uprights, 8 levels of storage, plus top. Nine inch platform to have two 48 wide and two 36 wide Storage Units. Large platform to have 18 deep shelves for two boxes deep, per shelf. Three 42 wide units and one 36 wide unit. Carriage to have 18 deep units, back-to-back, with full-height metal back, two 42 wide and two 36 wide storage units. All shelves to be vertically adjustable without bolts, nuts and sway braces. All shelves to be rated for 300 pounds per level. As per Drawing 154620 A2. Delivered and Installed. VENDOR PERFORMANCE INSTRUCTIONS: The BVAMC will prepare the area where the products will be installed before the installation can begin. The successful bidder shall coordinate the shipment date, delivery and installation with the BVAMC Engineering Department and/or Interior Designer no less than ten (10) business days before the confirmed installation date. The installation date shall be confirmed by the BVAMC in writing. Any storage and/or holding fees will be at no additional cost to the BVAMC. All products must be American made and on GSA Schedule. SECURITY: All contractors/installers must report to The BVAMC Annex Building located at 807 20th Street South, Birmingham, Alabama 35233 to be checked in and badged before entering the facility and must return to same location to be checked out and return badge upon completion of their day. The badge must be worn at all times while working at the facility. The Contractor will be responsible for ensuring compliance by its employees with the security regulations of the Veteran s Administration where work is performed under this Contract. Contracted employees will have to self-register at https://www.tms.va.gov/learning/user/SelfRegistrationUserSelection.do. Once registered, enroll in class va10176 VA Privacy and Information Security Awareness and Rules of Behavior. (See attached Contractor Rules of Behavior) No unauthorized Photography or Video on medical center premises. Per VA Regulations 1.218(b) (23) Use of recording devices of any kind to record patients or employees is strictly prohibited. Upon completion or termination of the applicable contract(s) or agreement(s), return and/or destroy, at Covered Entity s option, VA information gathered, created, received, or processed during the performance of the contract(s) or agreement(s). No data will be retained by Business Associate, or contractor, subcontractor, or other agent of Business Associate, unless retention is required by law and specifically permitted by Covered Entity. As deemed appropriate by and under the direction of Covered Entity, Business Associate shall provide written assurance that all VA information has been returned to Covered Entity or destroyed by Business Associate. If immediate return or destruction of all data is not possible, Business Associate shall notify Covered Entity and assure that all VA information retained will be safeguarded to prevent unauthorized Uses or Disclosures. Prior to termination or completion of this contract, contractor/subcontractor must not destroy information received from VA, or gathered/created by the contractor in the course of performing this contract without prior written approval by the VA. Any data destruction done on behalf of VA by a contractor/subcontractor must be done in accordance with National Archives and Records Administration (NARA) requirements as outlined in VA Directive 6300, Records and Information Management and its Handbook 6300.1 Records Management Procedures, applicable VA Records Control Schedules, and VA Handbook 6500.1, Electronic Media Sanitization. Self-certification by the contractor that the data destruction requirements above have been met must be sent to the VA Contracting Officer within 30 days of termination of the contract. WARRANTY: Successful bidder shall correct any Non-conformance at no additional cost to the BVAMC. PLACE OF PERFORMANCE: Birmingham VA Medical Center Dental Clinic located at 700 South 19th Street, Birmingham, Alabama 35233. A receiving area is located at the back side of the medical center on the 7th Avenue. The successful bidder shall have the delivery; unloading and installation properly coordinated with the BVAMC Interior Designer or approved designee before the product arrival at the jobsite. Any additional cost associated with this contract not previously included in the bid shall be at the successful bidders cost with no additional cost to the Birmingham VA Medical Center. DELIVERY AND WAREHOUSE INSTRUCTIONS: Deliver materials to jobsite in manufacturer's original sealed containers with brand name marked there on to protect materials from damage. Package to prevent damage or deterioration during shipment, handling, storage and installation. Maintain protective covering in place and in good repair until removal is necessary. Deliver products to jobsite only when the site is ready for installation work to proceed. Store products in dry condition inside enclosed facilities. INSTALLATION: Protect products against damage during field handling and installation. Protect adjacent surfaces/finishes as much as possible during installation to limit wall damage. The successful biddor shall remove all debris INSTALLATION HOURS OF PERFORMANCE: Hours of performance shall be scheduled between Mondays through Friday from 8:00 AM to 4:00 PM, excluding federal holidays. Federal Holidays observed by the VAMC are: New Year s Day, Martin Luther King Day, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. After hours and/or holiday performance schedules are not approved without written approval from the BVAMC. RESPONSE TIME: Installation shall be coordinated at least ten (10) business days in advance with the BVAMC Interior Designer or assigned designee. Installation shall be completed no later three (3) months of the executed purchase order. PRICE PROPOSAL: Include firm fixed cost for product, freight and installation on separate line items as noted on the excel spreadsheet. Any additional cost shall be specifically called out as such on a separate line item on the excel spreadsheet.
