Description

The Department of Veterans Affairs is pleased to release the attached request for quote for Chlorine Injection System Maintenance at the Coatesville Veterans Affairs Medical Center, 1400 Blackhorse Hill Road, Coatesville, PA. Submit quotes in accordance with FAR provision 52.212-1 Instructions to Offerors incorporated by full text in the RFQ, to include: Submit quote on the SF 1449; Provide evidence of Possession of Pennsylvania Department of Environmental Protection (PADEP) Operator Certification for Small Ground Water Systems; Complete the Price/Cost Schedule in Section B; Provide a completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically); and Provide past performance information to include three recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with e-mail addresses, telephone numbers, and other relevant information). Email quotes to Mr. John Evans at john.evans15@va.gov no later than 4:30 PM on 24 February 2017.
