Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures; and supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-87, dated March 07, 2016. Solicitation VA262-17-Q-0451 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. In order to meet the requirements set forth under Class Deviation by the Department of Veterans Affairs Veterans First Contracting program (VFCP 2016) issued on July 25, 2016 and FAR Part 19.1405, Service-disabled Veteran-owned Small Business (SDVOSB)/VOSB Set-aside Procedures, specifically 19.1405(b), this procurement will be issued under as a Total Service Disabled Veteran Owned Small Business Set-Aside. NAICS Code is 339112 and the Small Business Administration s (SBA) size standard 1000 Employees. The Government is seeking a contract to acquire an ULTRASONIC ASPIRATOR CONSOLE and components. This solicitation contains a brand name or equal to request for the following systems and components: 1. 5450-850-000 Sonopet 110V Console with Foot Pedal and Pole 2. 5450-850-410 Sonopet Cart 3. 5450-820-000 Sonopet Angled Handpiece, Universal 4. 5450-800-039 Torque Wrench 90 Degree, Universal 5. 5450-800-278 Sterilization Tray 7mm, Universal See attached SF 1449 Solicitation for further details. Place of Performance: VA Loma Linda Healthcare System (VALLHS), 11201 Benton Street, Loma Linda, CA 92357. Period of Performance: Within 45-60 days of placing order or sooner if available. Provisions 52.212-1, Instructions to Offerors Commercial and 52.212-2, Evaluation Commercial Items are applicable to this acquisition. Furthermore, Provision 52.212-2 is utilizing the Lowest Price Technically Acceptable (LPTA) on an Acceptable/Unacceptable evaluation process.
