Description

This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Solicitation number VA262-17-Q-0428 is issued as a Request for Quotation (RFQ) and constitutes the only solicitation. It is the offeror s responsibility to monitor this site for the release of amendments (if any). This notice and the incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-95, effective January 19, 2017. This is a competitive solicitation; 100% set-aside for Service Disabled Veteran Owned Small Business (SDVOSB). The North American Industry Classification System (NAICS) code for this acquisition is 238330 Flooring Contractors and the small business size standard is $15 million. Offeror must be registered in the System Award Management (SAM) at https://www.sam.gov, in order to be considered for award. See Section B Schedule of Supplies/Services and Price/Cost in Attachment I for the contract line item number, quantity, and unit of measures. Description of services: The Government is seeking for a licensed Contractor to replace floor coverings at VA San Diego Healthcare System (VASDHS). As specified in the Statement of Work (SOW) in Attachment I Section B.3, the Contractor shall provide all resources necessary to accomplish all requirements in performing the removal of the existing carpet and installation of new floor coverings. Period of Performance: The work to be performed is anticipated to begin approximately on or before March 10, 2017 for a period of four (4) weeks. Place of Performance: VA San Diego Healthcare System, 3350 La Jolla Village Dr. San Diego, CA 92161 FOB Terms: Destination The provision at Federal Acquisition Regulation (FAR) 52.212-1, Instruction to Offerors - Commercial Items (Oct 2015), applies to this acquisition. See addendum to FAR 52.212-1 in Attachment I Section E (E.1) Solicitation Provisions. A similar provision at FAR 52.212-2, Evaluation Commercial Items is used. See evaluation procedures in Attachment I Section E (E.2). Offeror shall include a completed copy of FAR 52.212-3, Offeror Representations and Certifications Commercial Items, with its offer. The clause at FAR 52.212 4, Contract Terms and Conditions --Commercial Items, applies to this acquisition and full text may be accessed electronically at http://farsite.hill.af.mil. See addendum to FAR 52.212-4 in Attachment I Section C (C.1). The clause at FAR 52.212 5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. See Attachment I Section C (C.2). Additional contract requirements necessary for this acquisition is attached. See Attachment I - Section B (B.4) Special Contract Requirements. Defense Priorities and Allocations System (DPAS) and assigned ratings are not applicable to this acquisition. Offer is due on February 27, 2017 at 12:00 PM PST. All email attachments in response to this RFQ shall be in either Adobe or Microsoft Office format. Offeror is advised that the Government may be unable to receive other types of electronic files (e.g. compressed or zip files) or files in excess of ten (10) megabytes (MB). Upon receipt, a notification confirming receipt of email will be sent to the Offeror. It is the offeror s responsibility to ensure all required documents are included and completed as required by this solicitation.
