- This is a SOURCES SOUGHT NOTICE for market research purposes only to determine the availability of potential businesses with capabilities to provide the services described in the attached draft Statement of Work. Potential offerors are invited to provide information via e-mail to jaclyn.alverson@va.gov. All responses will be used to determine the appropriate acquisition strategy and whether or not a small business set aside shall be utilized for a potential future acquisition. If your company is capable of providing the services required of the attached draft SOW, please respond to this Sources Sought notice by Friday, February 17, 2017 at 12:00PM CST. Responses should include: (1) Business Name and Address; (2) Point of Contact Name, Phone Number, and E-mail Address; (3) DUNS and NAICS code; (4)Business Size SMALL or LARGE; (5) Type of Business: SERVICE DISABLED VETERAN OWNED, VETERAN OWNED, 8A, HUBZone, WOMEN-OWNED, etc. All businesses eligible to provide this service are encouraged to reply, as this information shall be used to determine potential set-asides for the above noted service. This is NOT A REQUEST FOR QUOTE or an announcement of a solicitation. This synopsis is for information and planning purposes only and is not to be construed as a commitment by the Government nor will the Government pay for information solicited. No formal solicitation document exists at this time, and no contract will be awarded from this announcement. Draft SOW: STATEMENT OF WORK Repair and Preventative Maintenance of Alcon Centurion Vison Phacoemulsification System, VA Green Bay CBOC BACKGROUND: The Alcon Centurion Vision phacoemulsification system is used for cataract surgery procedures. This system is used to emulsify and aspirate the lens within the eye using the system s ultrasonic and aspiration hand-pieces. The space previously occupied by the lens is filled with saline fluid before a new lens is surgically implanted. This phacoemulsification system is used for twelve (12) cataract surgeries per week at the Milo C. Huempfner VA Outpatient Clinic (Green Bay, WI). SCOPE: The Contractor shall furnish all labor, transportation, parts, and expertise necessary to provide necessary repairs and conduct preventive maintenance on an Alcon Centurion Vision phacoemulsification system. All travel expenses are included in the contract price. EQUIPMENT IDENTIFICATION: Alcon Centurion Vision phacoemulsification system S/N #: 1403123001X EE #: 88161 Location: Milo C. Huempfner VA CBOC 2851 University Ave Green Bay, WI 54311 PERIOD OF PERFORMANCE: The period of performance shall begin on the date of contract award. Services shall be scheduled directly with the VA Green Bay CBOC Biomedical Engineer. All services shall be scheduled and completed no later than March 7, 2017. REQUIRED TASKS: The Contractor shall provide the necessary manpower and supervision to properly execute the preventative maintenance (PM) and repair of the equipment covered under this contract. The Contractor shall furnish all tools and materials (e.g. service manuals, diagnostic software, etc.) required to maintain the equipment in top operating condition. The VA shall not provide tools, (test) equipment, service manuals, etc to the Contractor. The Contractor shall obtain, have on file, and make available to their FSEs, all operational and technical documentation which are necessary to complete the services required herein. The Contractor shall supply and install a new Connector Block that is fully compatible with the Alcon Centurion Vision phacoemulsification system. The Contractor shall supply and install a new Tray Arm that is fully compatible with the Alcon Centurion Vision phacoemulsification system. PM shall be performed by the Contractor s Field Service Engineer (FSE). The Contractor shall perform PM according to manufacturer s specifications. The Contractor shall follow the manufacturer s written procedures for PM and must document that the equipment covered herein has been inspected according to those procedures. All parts required for PM are included in the price of this contract. Any and all travel required by the Contractor to complete the work required herein is covered by this contract and shall not be separately charged to the Government. HOURS OF COVERAGE: Normal hours of coverage are Monday-Friday from 8:00am 4:30PM, excluding holidays. All work shall be performed during normal hours of coverage. Any exceptions/changes to the hours services are provided shall be arranged and approved in advance with the CO. The Contractor is not required to provide service on the following holidays, nor will the Contractor be paid for these holidays. The following holidays are observed by the Federal Government: New Year s Day January 1st Martin Luther King s Birthday Third Monday in January President s Day Third Monday in February Memorial Day Last Monday in May Independence Day July 4th Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day November 11th Thanksgiving Day 4th Thursday in November Christmas Day December 25th REPORTING: The Contractor s FSE shall contact the VA Green Bay CBOC Biomedical Engineer at 920-431-2697 (Andrew Yunker) to schedule services. Upon arrival at the VA Green Bay CBOC on the date services will be performed, the Contractor s FSE shall contact Andrew Yunker at the number listed to report. The Contractor s FSE shall also check-out at the end of the day, upon completion of services. DOCUMENTATION: Upon completion services (replacement of parts as annotated in Paragraph 4 and PM), the Contractor shall provide the following information in a written service report to the VA Green Bay CBOC Biomedical Engineer: The date of service; The model; The serial number; The location (building and room number) of the equipment services; The name of the FSE providing services; The services performed and any parts replaced or repaired This report shall be signed by both the FSE performing services and the VA Biomedical Engineer. Either a hard copy or an emailed copy of the service report shall be supplied within 48 hours of completion to the Biomedical Engineer. QUALIFICATIONS: To be considered eligible for consideration, the Contractor shall provide FSE s that are trained by the Original Equipment Manufacturer (OEM) on the equipment specified herein. The Contractor shall provide, upon request, evidence/documentation of factory certified service/maintenance training on the specific equipment covered herein for each FSE performing services. The Government reserves the right to accept or reject Contractor s staff for the rendering of services. Complaints concerning Contract Personnel performance or conduct shall be dealt with by the Contractor and the VA Biomedical Engineer with the final decision made by the Contracting Officer. CHANGES: The Contractor is advised that only the Contracting Officer, acting within the scope of the contract has the authority to make changes which affect the contract in terms of quality, quantity, price or delivery. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment shall be made in the contract price to cover any increase in costs incurred as a result thereof. CONTRACTOR S RESPONSIBILITIES: The Contractor shall maintain personal liability and property damage insurance prescribed by the laws of the Federal Government. He/she shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. He/she shall be responsible for any injury to himself/herself, his/her employees, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by him/her or his/her employees fault or negligence. Contractor shall comply with all VA safety standards, manufacturers/industry standards, FDA, OSHA, JCAHO and the latest edition of NFPA-99. SAFETY AND FIRE PREVENTION: In the performance of this contact, the Contractor shall take such safety precautions as necessary to protect the lives and health of the occupants of the building. Fire and safety deficiencies, which exist and are part of the responsibility of the Contractor, shall be immediately corrected. If the Contractor fails or refuses to correct deficiencies promptly, the COR may issue an order stopping all, or any part, of the work. The Contractor shall comply with applicable Federal, State, and local safety and fire regulations and codes, which are in effect during the performance period of the contract. The Contractor personnel shall follow applicable facility policies concerning fire/disaster programs.
