- This Attachment is a fill-able form which must be completed in accordance with the instructions detailed in the Sources Sought Announcement. This Sources Sought Announcement contains specific instructions to perspective vendors. Please read in its entirety. This is a Sources Sought Notice. In preparation for an upcoming procurement, the National Oceanic and Atmospheric Administration (NOAA), Eastern Region Acquisition Division, Norfolk, VA on behalf of the Office of Marine and Aviation Operations (OMAO) Health Services Division, Norfolk, VA, is conducting market research to determine potential sources for recurring procurements on the NOAA Fleet Formulary and Medical Inventory (See Attachment A) to NOAA's eighteen (18) vessels within one total contract solution. The North American Industrial Classification System (NAICS) code is 423450, Medical, dental, and hospital equipment and supplies merchant wholesalers. All types of businesses with the capability to fulfill the requirements in Attachment A - NOAA Fleet Formulary and Medical Inventory Checklist are encouraged to fill the form out accordingly. Additionally, interested vendors are to submit a Capability Statement, in response to this sources sought announcement. Submitted Capability Statements shall include the following: (1) Company name and address (2) DUNS number (3) Type of business (e.g., large business, small business, 8(a), veteran-owned small business, service-disabled veteran owned small business, HUBZone small business, small disadvantaged business, and/or women owned small business) as validated via the System for Award Management (SAM) (4) Company point of contact- name, phone, and e-mail address (5) Capabilities required: a. Attachment A, completed in its entirety. b. Additional information regarding vendor capabilities: 1. Address unmarked items in Attachment A, if necessary. 2. Can unmarked items be provided? c. Provide a brief statement which demonstrates the ability to provide the entirety of the NOAA Fleet Formulary and Medical Inventory (Attachment A) for NOAA's eighteen (18) vessels via one total contract solution. Interested sources shall submit an electronic copy of their Capability Statement and the completed Attachment A to Contract Specialist, Sarah Rall, via e-mail at Sarah.rall@noaa.gov no later than 1:00 PM EST on February 22, 2017. This is not a request for technical or cost proposals. The Government does not intend to award a contract on the basis of this Notice or reimburse for costs incurred by interested parties providing the information requested herein. Capability Statements will not be returned. Information received will be considered solely to make an informed decision regarding a potential procurement. Any questions or responses to this Sources Sought announcement shall be directed to Sarah Rall, Contract Specialist, via e-mail at Sarah.rall@noaa.gov. Responses to the Sources Sought announcement are not offers and cannot be accepted by NOAA to form a binding contract.
