- Attachment D- RFQ Questions Attachment C- Pricing Sheet in PDF Attachment C-Pricing Sheet Attachment B- Specifications Request for Quote SF 1449 (SB1341-17-RQ-0159) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and the accompanying attachments. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This combined synopsis/solicitation, SB1341-17-RQ-0159, is being issued as a request for quoted (RFQ) utilizing simplified acquisition procedures authorized by FAR Subpart 13.5. The Contracting Officer will not use nor shall this solicitation be subject to the formal source selection procedures described in FAR part 15. All communications and documents must include the solicitation number. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95. Contract Line Item Numbers: Please reference the attached solicitation documents. Specification: Please reference the attached solicitation documents. All solicitation documents are attached to this RFQ. Please reference all attachments for complete solicitation. The potential offeror shall submit a Quote, which must include the following: • Company name, address, phone numbers, POC email address; • DUNS number; • Completed Schedule of Prices; • Itemized Proposal including all minimum all specifications, installation, training, and 3 year warranty; • Completed Schedule of Prices; • Place of manufacturer; • Delivery Schedule
