Attachment 3- Publick Law 94-437 Attachment 4 - Price Schedule Statement of Work & Provisions and Clauses Attachment 1- Aftercare Tracking Sheet Solicitation - SF 1449 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is Solicitation(17-247-SOL-00032) and is issued as a Request for Proposal (RFP). Under this requirement, the Phoenix Area Office Indian Health Services intends to award a Firm Fixed Price (FFP) for contractor to provide residential adult dual diagnosis Treatment program and are seeking possible contractors to provide all personnel, supplies, material and facilities required to perform a culturally sensitive integrated behavioral health, alcohol and substance abuse delivery system of inpatient/ residential services to eligible American Indian/Alaskan Natives that reside in Phoenix Area of the Indian Health Service which covers the states of Arizona, Nevada, Utah, and California. The preferred place of performance would be within these four listed states. However, consideration of technical proposals from residential treatment programs in Oregon, Colorado and New Mexico will also be considered based on Native American treatment programs available in those states as well as consideration of specialized treatment services offered by program identified in those states. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72 effective January 30, 2014. The applicable North American Industry Classification Standard (NAICS) code is 623320 Residential Mental Health and Substance Abuse Facilities. The small business size standard is $15.0 Million. This acquisition is 100% Native American Indian Owned Business set-aside. The period of performance is 01 March 2017 through 28 February 2018. The solicitation includes four twelve month options. The propose location are in the states of Arizona, Nevada, Utah, California, Oregon, Colorado and New Mexico. The provision at FAR 52.212-1, Instructions to Offerors Commercial Items, applies to this acquisition. FAR 52.212-2, Evaluation-Commercial Items, applies to this acquisition. Please see the solicitation attached for additional clauses. Interested firms may identify their interest and capability to respond to the requirement and submit a proposal. - Questions regarding this solicitation must be submitted via email to edward.becenti@ihs.gov no later than 7:00 PM.
