Contracting Office Address Department of Veterans Affairs, Network Contracting Office 15 3450 S. 4th Street Trafficway, Leavenworth KS 66048 Description The Department of Veterans Affairs Consolidated Mail Outpatient Pharmacy program (CMOP) has a requirement for, but not limited to the following items: Description Fanfold Thermal Paper The National CMOPs located in Leavenworth KS, Chelmsford MA, Charleston SC, Dallas TX, Tucson AZ, and Hines IL have a requirement for fanfold thermal paper. The paper is used for the required patient safety information related to the pharmaceutical that has been prescribed to our Veteran patients. The paper must have the following characteristics: 8 width x 4 length direct thermal paper perforated every 4 Reflective black sensor bar must be carbon based, water based ink or UV cured no less than 10% reflectance at wavelengths of 950 and 640 mm Reflective black sensor bar must be on each sheet leading edge Caliper 3.4mils Fan folded (two sheet per fan fold) Paper should be white in color and text should appear in black after the thermal printing process Black timing mark 1/8 inch continually across the paper from left to right Non-coated 5000 count per box Must be no more than 80 boxes per pallet to prevent damage to the bottom row of paper Paper must be compatible with Datamax H-8308 printers Tucson CMOP paper must include 1 direct thermal print head cleaning card, 8 x 6 for each case of paper. Estimated Yearly Requirement: Charleston: 42,000 boxes Chelmsford: 19,200 boxes Dallas: 10,000 boxes Hines: 33,600 boxes Leavenworth: 27,040 boxes Tucson: 25,000 boxes Description - Shipping Labels The National CMOPs located in Leavenworth KS, Chelmsford MA, Charleston SC, Dallas TX, Tucson AZ, and Hines IL, have a requirement for shipping labels. The specific requirements for shipping labels vary for each CMOP. As a result, note the following requirements for each respective CMOP: Leavenworth CMOP labels must meet the following characteristics: To be used on TEC printer B-572 or Toshiba printer B-SX5T-TS22-QMR 2000 labels per roll/4 rolls per case - estimated yearly requirement: 1,152 cases Note: 4 cases of samples required to be submitted for acceptance 3.125 L x 4.25 W Direct Thermal Label Perforated between Labels 3 core Horizontal Feed Adhesive backing Inner Wound ROUNDED CORNERS 4.25 White 3.125 Feed Direction 1/8 gap between labels Label inner wound Chelmsford CMOP labels must meet the following characteristics: To be used on the Datamax-O Neil model M-4308 printers 1500 Labels per roll/ 6 rolls per case estimated yearly requirement: 1,400 cases Note: 1 case of samples required to be submitted for acceptance 2 L X 4 W Direct Thermal Label Color: White Material: Fade Resistance Direct Thermal Style: Strip and Perforated Liner Width: 4.125 Gap between Labels:.125 Core Diameter 3 Perforation between labels Hines CMOP labels must meet the following characteristics: To be used on Datamax Model M4308 printers 500 labels per roll/6 rolls per case estimated yearly requirement: 4,800 cases Note: 1 case of samples required to be submitted for acceptance 10.875 L x 2.25 W Round Corner Rectangle with slit/notch @ 5.375.50 gutter High Sensitivity Direct Thermal Adhesive Auto application 3.0 core 5 and 7/16 to gap 5 and 7/16 from gap 2.25 Post office will not deliver without proper postage Use this label to mail refill requests back to us Feed direction Tucson CMOP labels must meet the following characteristics: To be used on the SATO CT4081 printers 1500 labels per roll, 6 rolls per case estimated yearly requirement: 5,000 cases Note: 1 case of samples required to be submitted for acceptance 4 L x 2 W Liner width: 2.125 Gap between labels:.125 Core diameter: 3 Material: white fade resistant pharmacy direct thermal On the back side of the liner there must be a solid black strip 1/8th of an inch in width along the leading edge of the label not to exceed a variance of 1/64th of an inch either direction from the leading edge. Liner must be perforated in the center of the.125 gap Adhesive on the back of the label must be permanent type adhesive so that once placed on our poly mailers it cannot be removed Labels must be placed on roll with the label facing out. We request these labels be delivered on the first work day of each month. 4 2 Label rolled outside Charleston CMOP labels must meet the following characteristics: To be used on the Datamax I Class 203 DPI printers 1000 labels per roll/4 rolls per case estimated yearly requirement: 4,000 cases Note: 4 cases of samples required to be submitted for acceptance LABEL COATED THERMAL WHITE Label 5.5"L x 2.25"W Outer wound Core size: 3" 1/8" gap between labels Label must be centered on the label liner UV Coated High Sensitivity Direct Thermal Facecoat Acrylic/Rubber blend adhesive Important to adhere well to polyfilm bags and Styrofoam coolers Blank White; no imprint 5.5 2. 25 Dallas CMOP labels must meet the following characteristics: To be used on Datamax printers 1000 labels per roll/4 rolls per case/80 cases per skid estimated yearly requirement: 12,480 cases Note: 2 cases of samples required to be submitted for acceptance 5.5 L x 2.25 W White direct thermal labels Not perforated or splicing 3 inch core Easy pull off adhesive backing Outer wound Horizontal feed Kraft bleached liner Thickness 0.0225 to 0.0027 UV top coat to prevent marking on the label from handling Shelf life 12 months 5.5 2.25 ˝ gap between labels Feed direction The purpose of this sources sought notice is to evaluate industry availability and obtain statements of capability from any vendor who can supply these items, particularly SDVOSB and VOSB. Note: Sources sought responses will NOT be shared with other industry suppliers and will solely be used as part of the contract planning process.
