Page 2 of 2 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA250-17-Q-0204 and is issued as a request for quotation (RFQ) in accordance with FAR Parts 12 and 13.. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The associated NAICS code is 339114 Dental Equipment and Supplies Manufacturing and small business size standard is 750 employees. The requirement is for High and Low Speed Electric Dental hand-pieces for use at the Louis Stokes Cleveland VA Medical Center. Delivery location is the Louis Stokes Cleveland VA Medical Center, 10701 East Boulevard, Cleveland, OH 44106-1702. This announcement constitutes the only solicitation; a formal, written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. Background: VAMC Cleveland is requesting the Procurement of new High Speed and Low Speed Dental hand-pieces for the dental service at VAMC Cleveland. All instruments and equipment requested are listed below. These devices will be used during all current dental procedures currently conducted at VAMC Cleveland with the exception of Oral Surgery. VAMC Cleveland Dental Service is seeking standardization of all current hand-pieces in order to assist SPS personnel in proper cleaning and disinfection of hand-pieces. Under VHA Directive 1116, the manufacturer s Indications for Use (IFU) must be followed as written.
