Description

The Durham VA Medical Center intends to enter into Brand Name combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested, and a written solicitation will not be issued. Quotes shall reference VA246-17-Q-0521_1. This request for Quote (RFQ) document and incorporated provision and clauses are those in effect through Federal Acquisition Circular 2005-83. The Network Contracting Office 6-SAO East intends to award a firm-fixed price contract for Blue Chip Medical Supreme Air Low Pressure Mattress System (#9600-SP-84). Purchase order required by 3-1-17 to replenish stock. #9600-SP-04 Blue Chip Medical Supreme Air Low Mattress System includes: Supreme Air Model 9600 Mattress Self-sealing utilizing TPU Thermal Polyurethane material. Ventilated bladders include 40 air holes in sacral and torso Dual bladder design. Four bladder heal section Non-Kinkable hoses Heavy duty bladder anchors Twist CPR Valve. CPR rapidly deflates mattress for medical emergencies. Transport locking device remains inflated during a power outage or for patient transport. All material used are anti-microbial and fire retardant, and meet all fire safety and infection control standards. Mattress dimensions 36 X 84 X 8 Supreme Air Model 9600 Pump Solid State Pump Dual PC boards Adjustable settings Quiet high volume pump, durable, linear design, aluminum casing Low pressure alert Auto alarm system Pump includes 2 modes Dual Fuse Easy access filter. Heavy Duty rubberized brackets The North American Industrial Classification System (NAICS) code for this procurement is 337910 with a small business size standard of 500 employees. This procurement is a SDVOSB (Service Disabled Veteran Own Small Business), VOSB (Veteran Owned Small Business) and SB (Small Business) set aside. All eligible offerors may submit quotes. All offerors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award of a Federal Contract. The record must be complete and not missing elements such as representations and certifications. This award will be made using simplified acquisition procedures. No solicitation will be made available. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. A determination by the Government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the Government. Question concerning this solicitation shall be addressed to Natalie Bennett, Contract Officer, and emailed to natalie.bennett@va.gov. All questions or inquires must be submitted no later than 8:00 am Eastern Standard time on February 23, 2017. No phone call will be accepted. Any amendments to this RFQ will ONLY be available on the Federal Business Opportunities website (www.fbo.gov). The Government will award a contract resulting from this RFQ to the responsible offeror whose quote is the lowest price technically acceptable. Quotes are due by 4:00pm. Eastern Standard Time on February 23, 2017. Quotes received after this date and time will be considered late in accordance with 52.212-1(f) and will not be evaluated. Quotes shall be submitted via email to Natalie.bennett@va.gov.
