STRAIGHT PACH PROBE 10 EA 930.15 EA 9,301.50 Stock # HS0900.2236A: PRISM,GOLDMANN 200 EA 128.00 EA 25,600.00 MEASURING Jesse Brown VAMC is currently using only Haag-Streit tonometers, and their tonoprisms are specifically designed for their devices, with the correct weight and dimensions. Jesse Brown VAMC is currently required to have the tonoprisms processed / sterilized by the SPS department, where they are following the manufacturers guidelines. Given that Jesse Brown VAMC is currently using Haag-Streit tonoprisms, it would be difficult to keep separated an additional manufacturer, as they would have a different sterilization protocol, hence leading to confusion and possible incorrect disinfection. These prisms are used to measure the intraocular pressure of glaucoma patients. These probes are for use with a preexisting instrument that is functioning well, we simply need more probes to ensure that the VA is meeting a VA directive on disinfection. These probes are used in conjunction with the current pachymeter to capture the thickness of the cornea on all of our glaucoma patients. Authority: ORDER AGAINST FSS: Authority of the Multiple Award Schedule Program, Title III of the Federal Property and Administrative Services Act of 1949 (41 USC 251, et seq.); Title 40 USC 501, Services for Executive Agencies; and Section 803 of the National Defense Authorization Act of 2002 (PL 107-107) and implemented by FAR 8.405-6. Mark the applicable exception: X An urgent and compelling need exists, and following the procedures would result in unacceptable delays(FAR 8.405-6(a)(1)(i)(A); Only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized; (FAR 8.405-6(a)(1)(i)(B)) or In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order and the original order was placed in accordance with the applicable Federal Supply Schedule ordering procedures. The original order was not previously issued under sole-source or limited-sources procedures. (FAR 8.405-6(a)(1)(i)(C) Rational/explanation to support cited authority: The Jesse Brown VAMC has an urgent and compelling need for these prisms and probes in order to treat patients using properly sterilized equipment.
