- The solicitation W912PA-17-T-0008 closing date has been extended until 03 March 2017, 2.00 p.m. Central European Time. The solicitation W912PA-17-T-0008 closing date has been extended until 03 March 2017, 2.00 p.m. CET. All other terms and conditions remain unchanged. When submitting your offer, it is mandatory that you complete the attached Request For Quotation with your price schedule on the places reserved for each item on pages 3 thru 11. Also you will need to: 1. complete blocks 17a, 17b, 30a, 30b, and 30c. on Page 1 2. Insert NAME OF TAX that shall be exempted, example: VAT, 21%, on Page 52 (clause 252.229-7001 TAX RELIEF) 3. Provide your active record number from the Business Register of the Netherlands Chamber of Commerce (www.kvk.nl) 4. Provide a free drafted Quality Control Plan (QCP), in English language. The plan shall include at least the following four items: (i) How the monthly inspections of services and rented containers shall be performed (leakage, closing top, outside check, wheels, etc.); (ii) How pick-ups shall be verified upon completion (i.e. coupons, work orders, etc.); (iii) The title of the individuals who will perform the inspections; (iv)The methods for identifying and preventing defects in the quality of service performed before the level of performance becomes unacceptable; and 5. Provide a free template written and signed statement of compliance on all applicable Host Nation (The Netherlands) Regulations in English language. The statement shall express that your company adhere to the following Laws of Environmental Conservation (Wet Milieubeheer), Chapter 10: (3)(a) Waste Collection Decree (Besluit Inzamelen Afvalstoffen - BIA) (3)(b) Decision for Grease-Oil Separators (Besluit vetafscheiders-olieafscheiders) (3)(c) Regulation for Collectors, Transporters, Dealers and Brokers of Waste (Regeling inzamelaars, vervoerders, handelaars en bemiddelaars van afvalstoffen - Regeling VIHB) (3)(d) Regulation on Separation and Segregation Maintenance (Regeling scheiden en gescheiden houden - RSGH); *** Detailed special instructions are on page 11, and Instructions to offerors are on page 16 thru 21 (clause 52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS).
