- SYNOPSIS DESCRIPTION: The Department of Veterans Affairs, Network Contracting Office (NCO) 19 Rocky Mountain Acquisition Center, 4100 East Mississippi Avenue, Suite 900, Glendale, CO 80246 intends to solicit sources to enter into one firm-fixed price indefinite delivery, indefinite quantity contracts to provide on-site ophthalmology cataract surgery services for its Veteran population at the Grand Junction VA Medical Center (GRJVAMC) located at 2121 North Ave., Grand Junction, CO 81501. The Contractor shall furnish all personnel to provide services necessary to perform on-site Ophthalmology Physician Services to eligible beneficiaries of the Department of Veterans Affairs Medical Center, Grand Junction CO (hereinafter referred to as VAMC). The contract physician (s) care shall cover the range of Ophthalmology services as would be provided in a state-of-the-art civilian medical treatment facility and the standard of care shall be of a quality, meeting or exceeding currently recognized national standards as established by the American Academy of Ophthalmology (AAO) http://one.aao.org/guidelines-browse. Services shall be for a one-year base period with four one-year pre-priced option periods for renewal. Reimbursement methodology shall be a per procedure contract, the Contractor will be reimbursed only for each Current Procedural Terminology (CPT) Codes completed. The solicitation package will be available for download on or about March 3, 2017. Proposals submitted shall be due no later than 4:00 p.m. mountain time on March 24, 2017. The applicable NAICS code is 621111 with a small business size standard of $10.0 million. This solicitation is a 100% Unrestricted; however, SDVOSB verified small businesses are encouraged to submit a proposal. Awarding preference will be given to SDVOSB and small businesses. The solicitation reference is VA259-17-R-0272, On-site Ophthalmology (Cataract Surgery) Services, and should be referenced in all correspondence/inquiries. All information for this solicitation will be posted on the Federal Business Opportunities (FBO) website at www.fbo.gov. No hard copies of the solicitation will be provided. Any amendments to this solicitation will be available at this website and offerors are advised that they are responsible for obtaining amendments to this solicitation at this website prior to the time of responding to the solicitation.
